In this role, the successful candidate provides clinical and administrative supervision to Care Managers providing care management services to patients enrolled in the Adult Health Home Program. The Supervisor oversees the Care Coordination, Care Planning and Change Management activities of the Care Manager. The Supervisor also ensures that Care Managers address unmet social service needs that impact on patient health outcomes.
Primary Responsibilities and Essential Functions
- Responsible for review of clinical documentation such as progress notes and encounter review of assigned Care Managers to ensure that patient assessments, initial and annual Comprehensive Assessments are completed within required time frames and meet NYS Health Home standards.
- Responsible for review of clinical documentation of assigned Care Managers to ensure that Care Plans are completed within required time frames, address the social determinants of health and include lifestyle behavior change goals that address health, behavioral health and chemical dependency problems.
- Responsible for monitoring the achievement of Care Plan goals, graduation processes and for providing feedback to Care Management staff regarding the implementation of appropriate case management interventions.
- Review and follow up Lost to Service progress notes, quality engagement attempts and critical event follow-up.
- Responsible for monitoring Care Managers’ productivity and for tracking the provision of billable, monthly core services and case conference review, scheduling and evaluating case closures and transfers.
- Provide direct one-to-one and group clinical weekly supervision for the Case Managers.
- Responsible for temporarily providing services to clients if an assigned Care Manager’s position is terminated, this is to ensure continued services to the clients until cases are reassigned.
- Provide support around case conferencing and coordinate incident reporting procedures for program.
- Conduct regular clinical chart reviews for clients’ services, ensuring quality of documentation and compliance with City, State and Federal requirements.
Qualifications

A Bachelor’s degree in human health services /social work

3-4 years’ experience as a care manager or care coordinator.

Strong clinical, administrative and organization skills required.

Preferred: Bi-lingual Spanish / Russian

Strong written and oral communication skills.

Ability to use Microsoft office programs and electronic health record systems.

Relevant experience in serving the population with SMI & chronic conditions, developmental disabilities, behavioral health, primary health care, or social services.

Working knowledge of interdisciplinary planning process and the development of an individualized patient centered plan of care.

Ability to deliver family-driven, youth-guided services.
