Position Summary 

The Behavioral Health Coordinator plays a key role in integrating behavioral health services within the primary care setting. This position supports providers and care teams by coordinating referrals, tracking follow-up care, and ensuring patients receive timely access to behavioral health support. 


Serving as a bridge between patients, primary care providers, and behavioral health specialists, the coordinator enhances access, continuity, and quality of care while supporting population health and integrated care initiatives. The schedule for this position is Monday- Friday 7:00 AM - 3:30 PM.


Key Responsibilities: 

  • Coordinate behavioral health referrals generated by primary care providers, ensuring timely scheduling and follow-up. 
  • Conduct patient outreach to encourage engagement in therapy, psychiatry, or counseling appointments. 
  • Communicate effectively with primary care providers regarding patient barriers, follow-up needs, and care progress. 
  • Document and maintain all outreach, communication, and coordination activities accurately in the electronic health record (EHR). 
  • Track completion of behavioral health visits and report outcomes to support quality improvement initiatives. 
  • Provide education to patients and staff about available behavioral health services and community resources. 
  • Support care management and population health initiatives by identifying patients in need of behavioral health interventions. 

Qualifications: 

  • Education: High school diploma or equivalent required; additional education in psychology, social work, or healthcare preferred. 

Experience: 

  • Minimum of 1–2 years of experience in healthcare, case management, or behavioral health coordination. 
  • Experience in a primary care or integrated care environment is strongly preferred. 

Skills: 

  • Excellent communication and interpersonal skills with the ability to build trust and rapport. 
  • Strong organizational and documentation abilities. 
  • Proficiency in EHR systems and Microsoft Office Suite. 
  • Knowledge of community behavioral health resources and referral processes.

Salary Description
$24.00/hr.
