Who are you?

You are a caring professional who is dedicated to providing oversight of case management services by working closely with a team of case managers as they guide their clients from all walks of life, which includes but is not limited to homelessness, substance use, primary mental health diagnosis and/or co-occurring disorders, and incarcerated population.

Guide your team of client advocates as they attend treatment plan development and review meetings, coordinate and monitor appropriateness of all services, examine and recommend new programs, and provide assistance in crisis situations to stabilize their clients. Ensure your team of case managers is providing exceptional client care, consultative services and/or promotion of the coordination and development of mental health and/or substance abuse services within the community. 

How you'll make a difference in this role:

Under the guidance of the Director of Clinical Services, you will provide supervision and administrative oversight for a team of service providers. This supervision may include: appropriate assessment resulting in a diagnosis, development of an appropriate treatment plan, supervision of the therapeutic intervention/modalities of treatment, licensure/credential advancement, as well as professional/programmatic growth.

  • Assign and monitor staff case lists, productivity and other related tasks
  • Develop work schedules; monitor and address staff performance, productivity, and documentation
  • Monitor outcomes and fidelity to assigned programs in accordance with organizational policies and procedures
  • Coordinate services necessitated by emergency calls; ensure follow-up services at FRC and/or referrals to other community resources; monitor admissions, assignments, and discharges of persons served. 
  • Participate and conduct trainings and development to include types of intervention/modality of treatment provided to persons served
  • Collaborate in an interdisciplinary team to plan, develop, implement, and evaluate current and new programs as necessary
  • Participate in the recruitment, selection, hiring, and orientation/training of staff as needed

You have:

  • GED/HSD with a combination of a minimum of 5 years of field experience (preferably in community behavioral health), life experience, or relevant education 

                                  OR

  • Bachelor's Degree in Social Work/Psychology with an LSW/LPC or higher
  • A valid driver's license and the ability to maintain auto insurance, as the position requires the capability to transport clients in your own personal vehicle within the community
  • Ability to travel between the counties we service including: Allen, Auglaize, Hardin, and Hancock
  • CPR/First Aid Certification or willingness to obtain one and willingness to submit background check and fingerprinting
  • Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously.
