Patient Placement Center Nurse supports care for Providence St Joseph Health (PSJH) patients by evaluating and coordinating the patient intake process. He/she will be involved in taking and triaging calls, and facilitating inbound and outbound transfers for PSJH facilities. In this role, he/she will act as a liaison between physicians, hospitals and other care providers who seek to refer, schedule admission or transfer patients into the hospital. He/she will ensure timely medical communication for patient referrals and transfers to/from the facility. He/she shall employ critical thinking to integrate objective data with knowledge gained from preliminary/initial information with the ongoing assessment process, including the objective and subjective experiences and patient health care history for optimal placement. In addition to core duties, the RN serves as a clinical resource for the team, providing guidance on clinical concerns, level of care determination, and regulatory considerations. They also support onboarding and training of new staff, helping reinforce consistent practices across the teams.

Required Qualifications:
- California Registered Nurse License upon hire.
Preferred qualifications:
- Bachelor's Degree Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing,
- Minimum of 2-5 years nursing experience.
- Working knowledge of Epic and Bed management system
