Job Title: Appointment Transporter

Supervisor: Appointment Specialist 

Department: Residential

FLSA Status: Hourly

Summary: Responsible for transportation to all appointments and coordinating with the Appointment Specialist about information for the appointment.    


Essential Duties

  1. Transport clients to all appointments.
  2. Communicate appropriate information to doctors.
  3. Ensure Medical Information Form is completed at the appointment.
  4. Notify Pharmacy Specialist immediately if a new medication is ordered.
  5. Assist with printing compliance paperwork for the compliance label books.
  6. Complete Individual Specific Trainings as they are updated.
  7. Track and monitor weights and calorie counts.
  8. Participate in the ER on call rotation.
  9. Assist participants with personal care when transporting them to scheduled appointments.
  10. When representing KCARC, communicate with professionals in a knowledgeable and positive manner. 
  11. Maintain updated folders with information that can be used as a quick reference tool.
  12. Report vehicle maintenance problems to designee immediately.
  13. Maintain a positive, professional attitude at all times.
  14. Maintain neat, clean, professional appearance.
  15. Administer medications as directed.
  16. Must be able to frequently lift and/or move up to 75 pounds independently and over 75 pounds with assistance.
  17. All other duties as deemed necessary.
  18. Additional hours if needed are mandatory

Requirements

Qualifications:  

  1. Maintain reliable transportation available for use during working hours, valid driver’s license and provide proof of automobile liability coverage.
  2. Possess an excellent driving record.
  3. Obtain and maintain KCARC authorization to drive agency vehicles.
  4. Maintain flexible schedule to meet the demands of the department.
  5. Must have a working understanding of HIPPA and maintain participant’s privacy. 
  6. Maintain good hygiene.
  7. Communicate effectively both verbally and in writing.
  8. Use tact and empathy when dealing with people of varied backgrounds.
  9. Establish effective working relationships.
  10. Work independently and in a team environment.
  11. Be proficient in Windows and Microsoft Office products (Computer literate).
  12. Accept responsibility and demonstrate leadership.
  13. Be able to effectively work in a sometimes fast paced environment. 

* Any changes in qualifications status must be reported to Human Resources immediately.


Education and/or Experience:  

High School diploma or GED equivalent required. Experience working with individuals with developmental disabilities is preferred. Knowledge of medical terminology preferred. 


Certifications/Licenses/Registrations: 

Medication Administration: Core A and Core B

CPR/First Aid


Physical Demands:  

The employee must be able to perform the essential duties of this job.


Work Environment: 

While performing the duties of this job, the employee is occasionally exposed to fumes, airborne particles and outside weather conditions. The noise level is usually moderate but occasionally loud. Use of Personal Protective Equipment such as vinyl or latex gloves is occasionally required.
