POSITION: Behavioral Health Case Manager
DEPARTMENT: Behavioral Health Department
LOCATION: 1575 Blue Hill Avenue, Mattapan, MA 02126
REPORTS TO: Behavioral Health Director
TYPE: Full-Time
SALARY: Negotiable
JOB SUMMARY:
We are seeking a professional and efficient Behavioral Health Case Manager for our behavioral health team. The objective of this position is to welcome our patients with a friendly and polite demeanor while assisting with tasks such as assessing and addressing patient’s needs and collaborating with community resources. It is imperative that this individual possess impeccable customer service skills and is knowledgeable in cultural competency and provides patient centered treatment.
Responsibilities:
- 75 % Case Management/Care Coordination
- · Complete a needs assessment for each referred patient to identify areas to be worked on in case management
- · From the needs assessment, work with patient to prioritize goals and develop a treatment plan
- · Follow up regularly with patients based on need (should be a minimum of monthly)
- · Provide resources to patients to assist with housing, monetary benefits (SSI/SSDI, EAEDC/TAFDC, SNAP, utility assistance, etc.), food insecurity, immigration, legal services, furnishings, education, transportation, childcare, and any other identified needs
- · Assist patients with paperwork for housing, immigration, and other benefits or programs
- · Report any concerns for child abuse, elder abuse, or abuse of people with disabilities to appropriate agencies
- · Connect patients to services within the health center which may help to meet their needs – including Behavioral Health, PrEP/HIV Navigator, Nutritional Counseling, etc.
- · Services should be provided in a culturally and linguistically appropriate way
- · Be skilled in de-escalation strategies and provide patient centered care
- · Be the liaison for BH and community resources
- · Connecting patients to community resources
- 25% Administrative
- · Document needs assessment and treatment plan in electronic medical record
- · Document each patient contact (phone or in person) in electronic medical record
- · Update treatment plans every three months
- · Maintain copies of all paperwork completed to be scanned into electronic medical record
- · Maintain caseload of 30-40 patients at a time
- · Attend supervision for regular review of assigned cases
- · Ensure that there is a signed Release of Information for any collateral contacts, care coordination, or family contact being done on patient’s behalf
Requirements
· BA/Masters level Social work or Counseling/Psychology degree.
· Minimum of one to two years relevant case management experience.
· Bilingual/bicultural in Haitian Creole or Spanish strongly preferred.
· Excellent communication skills with staff and patients.
· Experience in working with diverse culture preferred.
· Excel, outlook and EMR experience preferred.
· Comfortable working with patients remotely as needed
