Pay Range: $27.55-$31.65 plus shift differentials

Full Time -Evening Shift

GENERAL SCOPE: Provides nursing care to meet the needs of residents according to resident's care plan and per federal, state and JHR procedures. Directs and supervises the work of nursing staff members assigned with them.  Maintains the highest degree of quality care.

EDUCATION / QUALIFICATION: Graduate of an approved school of practical nursing with current New York State Practical Nursing license.  Possess good organizational, leadership and communication skills.  By law is responsible for their acts and those acts they delegate to others.   

ESSENTIAL FUNCTIONS:

  • Ensures resident care plans are being followed by carrying out appropriate nursing interventions, to achieve progress toward expected outcomes.
    • Supervises staff to ensure that patients’ care plans are followed.
  • Provides direction regarding CNA and LPN assignments.
  • Assists in data collection related to resident care, as directed by resident care plans and physician orders.
  • Documents response to resident care plans. Completes thorough documentation of residents’ response to their plan of care per unit guidelines, including pain management, care pathways and wound documentation.  Reviews records for accuracy and completeness of data.
  • Uses sound judgment and critical thinking to prioritize and follow the resident care plans, consistent with the residents’ care needs and physician treatment plans.
  • Transcribes and carries out physician's orders and integrates them into the resident's care plan.
  • Assists in providing direct personal care to residents, as needed, and assists with therapeutic and diagnostic procedures.
  • Collaborates with other health team members in providing care. Resolves resident problems and needs by utilizing multidisciplinary team strategies.
  • Helps maintain residents at their highest level of functioning by using rehabilitative nursing principles.
  • Provides and documents patient/family education, based on diagnosis and treatment plan in accordance with patient/family readiness and ability to learn.
  • Participates in discharge planning.  Completes appropriate paperwork and educates patient/family about home care needs.
  • Ensures that the nursing supply inventory is maintained.
  • Communicates change in resident's condition by notifying the nurse manager or supervisor. Communicates changes in residents’ conditions to all staff and families, as condition warrants.
  • Maintains pert sheet; records in residents’ medical records and gives report to on-coming shift.
  • Acts as the nurse in charge who coordinates the daily operations of the unit. Assists the Nurse Manager to coordinate, schedule, assign and audit the clinical operations of the unit.
  • Assists with MD day by making rounds with MD and transcribing orders.
  • May accept telephone orders by physicians.
  • Maintains residents' confidentiality.
  • Assists in data collection related to residents care as directed by individual care plans and physician orders and documents per protocol.
  • Intervenes during behavioral crisis situations, which may include the use of emergency physical interventions and/or problem solving.
  • Facilitates skills instruction and education for residents both individually and in group settings.
  • Completes assessments on residents and notifies RN Manager when ready for review and sign off.
  • All other job duties as assigned.

 EXPERIENCE: Six (6) months to one (1) year experience as a staff nurse.  One (1) year clinical experience with individuals with cognitive disorders preferred.  Long-term care experience preferred.
