Pay Range: $34.65-$39.81 plus shift differentials (Pay information will vary depending upon relevant experience for the position)
Full Time- Evening Shift
GENERAL SCOPE: Provides nursing care to meet the needs of patients according to patient's care plan and per Federal, State and JHR procedures. Directs and supervises the work of nursing staff members assigned with them. Maintains the highest degree of quality care.
EDUCATION / QUALIFICATION: : Current New York State Registered Professional Nursing license. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulation and guidelines that pertain to long-term care. Must possess leadership and supervisory ability to work with professional and non-professional personnel.
ESSENTIAL FUNCTIONS:
- Acts as the nurse in charge who coordinates the daily operations of the unit. Assists the Nurse Manager to coordinate, schedule, assign and audit the clinical operations of the unit.
- Supervises staff to ensure that patient’s care plans are followed.
- Assists primary nurses as needed to transcribe and carry out physician's orders and integrates them into the patient's care plan.
- Assists in providing direct personal care to patients as needed.
- Assists primary nurses with daily activities as needed.
- Helps maintain patents at their highest level of functioning by using rehabilitative nursing principles.
- Assists with therapeutic and diagnostic procedures. Gives medicines and treatments when needed.
- Coordinates schedules and staff assignments- ensures assignment sheets are prepared daily.
- Communicates patient's condition by notifying Nurse Manager. Communicates changes in patients’ conditions to all staff and families, as condition warrants. Ensures the maintenance of the pert sheet; records in patient's medical records and gives report to on-coming shift supervisor.
- Assists with admissions and discharges.
- Assists with medical rounds with provider and transcribing orders.
- Completes risk assessments.
- Maintains patients' confidentiality.
- Maintains a safe environment, conducive to positive health care and comfort, for patients and staff in accordance with established policies and procedures.
- Assists in data collection related to residents’ care as directed by individual care plans and physician orders and documents per protocol.
- All other job duties as assigned.
EXPERIENCE: Six (6) months to one (1) year experience as a staff nurse preferred. Long-term care, subacute experience preferred.
