Coordinates and manages diabetic education by assessing, planning, monitoring, and evaluating diabetic care taking into account individual needs, appropriate interventions and in accordance to established hospital policies.  Provides individual counseling, group education, community outreach activities and diabetes services, with the main focus on the out-patient setting- but may involve some in-patient diabetic education functions.

 

 Essential Duties & Responsibilities:

  • Assures all components of the diabetic referral (such as: Demographics, Insurance Info, Ht, Wt., Diagnosis, Plan of Care, A1C, Medical History, Current Meds, Recent Labs, etc.) are received to process the referral within 1 week or earlier as possible.  Arranges for an A1C to be obtained  if not done prior to their first visit.
  • Develops an appropriate rapport with clients and/or family or persons involved with the patient’s care, using interviewing techniques, as able, that fosters a trust and open communication with clients and/or family, or persons involved with the patient’s care.
  • Conducts effectively and accurately an individualized assessment on each scheduled patient prior to their group sessions or individual education/instruction
  • Provides appropriate and effective diabetic education in the individual and/or group setting according to established Program curriculum.
  • Outpatient Education and counseling is done in a timely manner considering current principles in diabetes self management.  Adapts diabetic program Methods and Materials to accommodate and provide education suitable for each client’s lifestyle and learning needs. (i.e./ Low Literacy Materials, Audio/Visuals, Magnifiers, Interpreter for Deaf or Different Language, etc.)  Coordinates effective group learning considering individual and/or group needs.  Instructs patients who need to learn self injection techniques or glucose monitoring  within 48 hours of receiving the referral, or as possible within the patient’s schedule/preference. Prepares charts and education materials and set up prior to the education provided.  As available, provides brief survival inpatient education, as appropriate, when an inpatient referral is requested.
  • Documents in an orderly, timely, and legible manner the patient assessment, interventions, education, evaluation, follow-up, and any other notes to providers involved in the patient’s care, avoiding dangerous abbreviations in documentation. Obtains written authorization and disclosure permission from the patient on the first visit. All areas of the Diabetes Self Management Education and Support Record are filled out according to each individual needs.
  • Coordinates appointments and referrals, as the patient allows, for services beyond the DSMES services. (i.e./ Dilated Eye exam, Foot exam, Dental exam, Mental Health-Social Service referral, Physical Therapy/Exercise, Food Banks ,etc.)  Communicates these interventions to appropriate staff and providers. Uses stickers on patient charts for reminder of eye or foot exams when needed.
  • Facilitates and documents follow-up sessions or opportunities with patients to obtain outcomes for the DSMES services
  • Distributes a mailing within one week of the follow-up luncheon or for those unable to come back for a follow-up session to obtain re-measurement of outcomes: weight, A1C, BS testing, behavioral goals, post test, blood pressure as available.  As a last resort, this can be done over the phone. Contacts the patients by phone beyond the 3-month follow-up at 6, 9, 12 months from the initial visit and documents the outcomes.  Sends a yearly letter to participants as a reminder of recommendations for follow-up care and asks participants if they are interested in coming to a Refresher Luncheon.
  • Maintains effective written and verbal communications  Intra, Inter departmentally and with public relations. Applies quality interviewing techniques such as:   Motivational Interviewing for good patient rapport and Crucial Conversations with employees/peers. Provides effective Hands-off communications. Mentors staff, students, colleagues and others through sharing of knowledge and skills. Providers are sent results of the patient education. Represents the hospital in a positive and professional manner.
  • Routinely plans and organizes resources and time to get the job done, by maintaining office supplies, glucose tablets, glucose meters, and handouts, etc.
  • Monthly, records Performance Improvement data and assists in Quality Improvement plans, and Departmental Quality Control Stats in a timely manner.  Keeps the records on the Excel data base for patients accurate and up to date, and files satisfaction surveys.
  • Maintains the DSMES services to current standards
  • Provides review and input for revisions and updates the Program & Manuals, as needed for educational curriculum, educational materials, using current Standards of Medical Care.
  • Maintains  accurate and timely records of personal employee record such as Registration, licensure, in-services, education and competency.
  • Obtains and maintains education requirements according to State of Ohio RN licensure law and in accordance with American Diabetes Association guidelines for a Certified Diabetes Education Program.  If a CDE (Certified Diabetes Educator), continuing education requirements are obtained to remain a CDE
  • Assists and supports the Director of Nutritional Services, as needed for other duties requested
  • Examples of other duties but not limited to:  Over site Committee, yearly P/P review, Community Diabetes Programs, Outlook Calendar Maintenance for Outpatient appointments, in-service education, news articles, and other program/ departmental needs.
  • Complies with federal, state, and local legal requirements by studying existing and new legislation; anticipating legislation; enforcing adherence to requirements; advising management on needed actions; ensures compliance with Joint Commission and other external and internal regulatory agencies.
  • Maintains the integrity of confidential patient information in accordance with HIPAA and hospital policy, including accessing, using and disclosing patient information relevant to the individual employee’s job duties.  Also maintains confidentiality of other customer and hospital business affairs.
  • Adheres to the standards of Magruder Hospital’s corporate compliance policies and seeks guidance and training when necessary to resolve questions about the policies.  Reports violations in accordance with corporate compliance and organizational standards.
  • Attendance is an essential function of this position.  Reports to work on time and as scheduled; Shows respect for co-workers by following established break times;   Keeps personal conversations and telephone calls to a minimum.
  • Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.
