Job Overview:

360 Total Care is seeking a dedicated and dynamic Community Outreach & Enrollment Coordinator to lead senior enrollment efforts for our Day Habilitation Center. This individual will be the face of our senior program in the community, actively building relationships, promoting services, and guiding families through the enrollment process. The ideal candidate has experience working with seniors, understands the Council on Aging, and has a background in outreach, networking, and sales.

Key Responsibilities:


Client Recruitment & Enrollment

  • Recruit seniors and their families into the Day Habilitation Center.
  • Conduct intakes, explain program offerings, and guide families through enrollment paperwork.
  • Maintain a waiting list of prospective participants to ensure consistent program growth.

Community Outreach & Branding

  • Represent 360 Total Care at community fairs, health expos, and local events to promote services.
  • Set up staff information tables at libraries, senior centers, and partner organizations.
  • Develop and implement creative outreach campaigns to build awareness and referrals.

Partnership Development

  • Build and maintain relationships with the Council on Aging, referral sources, case managers, and community organizations.
  • Attend networking meetings, senior-focused events, and community planning sessions to expand visibility.

Program Promotion

  • Collaborate with the marketing team to support branding, flyers, and event campaigns.
  • Provide feedback from families and the community to improve outreach strategies.

Documentation & Compliance

  • Ensure enrollment documentation meets Council on Aging and Medicaid requirements.
  • Track intakes, maintain accurate records, and support reporting needs related to client enrollment.


Qualifications and Experience:


Education: 

  • Bachelor’s degree in social work, Human Services, Healthcare Administration, Business, or related field preferred.

Experience:

  • Minimum 3 years of experience collaborating with seniors in healthcare, adult day, or social services setting.
  • Experience with Council on Aging processes and Medicaid services strongly preferred.
  • Background in community outreach, networking, or sales is highly desirable.

Skills:

  • Excellent communication and people skills with families, seniors, and professionals.
  • Strong organizational and time management skills.
  • Comfort with public speaking and presenting in community settings.
  • Ability to track goals, meet recruitment targets, and maintain accurate enrollment data.

Other Requirements:

  • Must be an active, community-driven professional willing to attend events and  network regularly.
  • Valid driver’s license and ability to travel locally.
  • Proficient with Microsoft Office and open to learning client documentation software.

  • Base Pay: $24 per hour.

Schedule and Trial Period

  • Base Schedule: Monday through Thursday 9:00 am - 2:00 PM. (Must be able to flex schedule to attend networking events.)
  • 20 hours weekly with potential to grow hours as client base increases.
  • Trial Period: This role will begin with a 30-day trial period to ensure mutual fit and alignment. Upon successful completion, the position will transition into a long-term engagement. 
  • Successful completion of trial period requires 3 new enrollments in first 30 days.
