Join the Harris Center for Mental Health and IDD as a Care CoordinatorUnder the general direction of the Clinical Team Leader, or designated staff, the Care Coordinator performs clinical activities such as independent assessments and service interventions as directed by the treatment plan and/or identified consumer need(s). This position interfaces with the Treatment teams both inpatient and outpatient and authorizes the medical necessity of services, as indicated. 
This position is responsible for providing the following services: Crisis Intervention, Mental Health 
Assessments, case management and follow up, and liaison between the Agency and the community. 
PES interfaces with other programs in the Agency, law enforcement, the courts, and the community.

Your Role in Action

As a Care Coordinator for this unit, you will be:

The PES Care Coordinator provides crisis counseling and intervention to individuals to reduce 
symptoms, provide education and prevent admission to a more restrictive environment. 
The PES Care Coordinator collaboratively creates a treatment plan with the consumer and PES 
treatment team to outline treatment goals for stabilization.
The PES Care Coordinator completes reassessments with consumers daily to determine the least 
restrictive setting for the consumer to continue treatment.
The PES Care Coordinator monitors and coordinates transfers to hospitals and outpatient services.
The PS Care Coordinator completes mental health commitment paperwork when needed for the 
safety of the consumer.
The PES Care Coordinator educates family members so they may understand the individual’s mental 
health symptoms, treatment, and assists with referring to community resources. 
The PES Care Coordinator provides information on mental health, the importance of medications and 
treatment compliance, and other clinical related information to individual, family, and community as needed.

The PES Care Coordinator completes electronic documentation each workday as outlined in the PES 
Operational Guidelines. This documentation becomes part of the medical record of the Agency.
The PES Care Coordinator, when interacting with the community and colleagues, follows the key 
principles of a trauma informed approach (i.e. safety, trustworthiness and transparency, peer 
support, collaboration and mutuality, empowerment, voice and choice, cultural, historical, and 
gender issues.)
The PES Coordinator meets Divisional and program productivity standards.
The PES Care Coordinator performs related work as assigned and adapts to changing work 
requirements.
The PES Care Coordinator demonstrates ethical conduct in all professional activities.
The PES Care Coordinator complies with Agency policies and procedures and PES Operational 
Guidelines.
The PES Care Coordinator is available to provide any assistance not listed above to the Agency, 
Program Director, Clinical Team Leader, or staff as requested and required.

What qualifications you will need:

Education:

  • Bachelor’s degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.

Experience:

  • Minimum of one year experience in the mental health field, preferred.

 Other Key Qualifications

  • Must maintain a valid Texas Driver's License and be insurable under Agency policy.
  • The ideal candidate will be required to transport consumers using their personal vehicle and will be reimbursed for mileage by the Agency.

Knowledge, Skills & Abilities:

  • Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.
  • Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct.
  • Able to work collaboratively with others in a teamwork environment.
  • Effective time management and organizational skills. Able to execute standard word processing.
  • Must be able to work autonomously in a fast-paced environment.
