About the Department

A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
OR
A bachelor's degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.

Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.

Desirable Qualifications:

  • Two years of social service-related experience.

Position Duties

Expected starting salary is $33.45 to $44.66. Full salary range is $33.45 - $55.87. 

Under supervision of a Program Manager or Supervisor, and within Federal, State, and locally developed programs and policies, supports individuals diagnosed with a disability.  Identifies individual needs and county and community support services to address needs.  The services help an individual live independently in community settings and promote optimal health, safety, and community integration.  Will be working within a multi-disciplinary team setting.  Supports a diverse, respectful, and inclusive workplace. 

Minimum Qualifications


  • Provides assessment and case management for individuals served through State Plan, State Funding programs, and Waiver Programs.
  • Uses revised MnCHOICES assessment tool to determine the functional needs and abilities of the individual to determine eligibility for services. 
  • Works with a variety of funding programs/sources (County, State, and/or Federal) to meet the individual's needs.
  • Develops and writes support plans.
  • Advises individuals and their support networks of possible resources, rights and responsibilities under programs or services.
  • Works with individuals, team members, and providers who support the individual.
  • Refers individuals to other programs or agencies when appropriate.
  • Utilizes State and Local computer programs to document work for the county and the individual.
  • Maintains case records in accordance with State, Federal, and local guidelines.
  • Performs all other duties as assigned.

  • Knowledge of a variety of program and services for individuals with disabilities or related conditions, including knowledge of physical health, mental health, and chemical health needs.
  • Knowledge of the dynamics of individual and group behavior.
  • Knowledge of the principles, methods and techniques of social work.
  • Knowledge of social and economic problems.
  • Knowledge of community resources.
  • Knowledge of federal and state Social Service programs and laws.
  • Knowledge of data practices and confidentiality.
  • Skill in effective written and verbal communication.
  • Skill in the use of computers and technology.
  • Skills in individual and community advocacy.
  • Experience coordinating services for individuals and conducting assessments.
  • Ability to understand individual and family needs with empathy and objectivity.
  • Ability to exercise good judgment in recommending possible solutions for individual and family needs and when utilizing conflict management and advocacy skills.
  • Ability to establish and maintain effective working relationships with clients, co-workers, public officials, and citizens of the community.
  • Ability to contribute to a positive and healthy work culture.
  • Ability to organize efficiently and keep records systematically.
  • Ability to work with diverse populations.
  • Ability to serve as a liaison to community resources.
