Job Description:

General Summary

The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. The position is goal-oriented with a high level of accountability correlated to the continuum of care, quality of care and patient satisfaction.

Reporting Relationships

Reports to the Managerof Care Management.

Qualifications:

Education: Graduation from an accredited School of Nursing or college with current nursing licensure in the State of Maryland. BSN or Master prepared preferred.

Experience:Minimum of 3-5 years of relevant clinical hospital experience required as applicable to the position needs. Ongoing education, certification, and self-development to remain current with industry standards and business objectives related to hospital case management.

Compensation:

Pay Range: $40.61 - $60.96
