Hospice Social Worker Job Summary

• Responsible for provision of social work services to include explanation of services, crisis intervention, resource referral and emotional support and/or counseling to patients and families as needed.
• Ability to work in coordination with other disciplines at part of team to address chronic illness and end-of-life needs
• Complete psychosocial assessments for new patients and explain hospice services
• Participate in the hospice social work call schedule.

Hospice Social Worker Qualifications

• Masters Degree in Social Work.
• NC LCSW, OR LCSW Provisional license preferred. --Must obtain provisional license after hire.
• At least 1 year experience in social work required.
• Knowledge of end-of-life issues preferred.
• Reliable transportation and valid NC Drivers license required.
