Your Role as a Hospital Liaison

The Hospital Liaison serves as the vital connection between acute care settings and home health, ensuring patients experience a safe, smooth, and timely transition home. This role blends relationship management, transitional care coordination, and business development, supporting patients, referral sources, and care teams alike.

Key Responsibilities:

• Transitional Care Coordination: Facilitate the referral-to-admission process by collaborating with hospital case managers, discharge planners, physicians, and families to ensure all required documentation and clinical information are obtained and processed promptly.
• Weekend Admissions Support: Serve as the primary liaison for weekend referrals, admissions, and transitional care planning during designated hours.
• Patient Advocacy: Educate patients and families about home health services, ensuring an informed and seamless transition home.
• Relationship Development: Build and maintain strong partnerships with hospital leadership, case management teams, physicians, and specialists to increase awareness and utilization of home health services.
• Clinical Collaboration: Partner with internal clinical teams to align on patient care needs and ensure continuity of care across hospital, home health, and community settings.
• Community & Physician Education: Provide ongoing education to physicians, hospital staff, and families regarding home health capabilities, clinical programs, and transitional care benefits.
• Performance Goals: Consistently meet or exceed referral and admission targets by leveraging strong networks and proactive follow-up.

Qualifications

• Established hospital relationships or referral network within Hillsborough/Pinellas counties (required)
• Clinical or case management background (RN, or LPN) required
• Prior home health, post-acute, or hospital liaison experience (preferred)
• Excellent interpersonal communication, presentation, and problem-solving skills (required)
• Proficiency in Microsoft Office (preferred)
• Detail-oriented, organized, and able to work independently in a fast-paced environment (required)
• Valid driver’s license, auto insurance, and reliable transportation (required)
