Our client is seeking an RN Case Manager to provide field-based hospice care in Victoria, Texas. This role is part of a growing and experienced hospice network known for scaling care teams and expanding into new markets. They’re looking for someone reliable, compassionate, and hands-on, ready to make a difference.
What You’ll Do
Deliver compassionate hospice care in the field, managing a caseload of patients.
Coordinate and communicate with the interdisciplinary care team, patients, and families.
Ensure compliance with hospice standards and maintain accurate, timely documentation.
Participate in on-call rotation (extra pay provided).
Complete occasional after-hours visits (paid per visit).
Play a key role in providing stability and continuity of care to the community.
What You Need
RN license in Texas (required).
Hospice experience preferred, but not required (training available for nurses with the right mindset).
Strong clinical skills and comfort with fieldwork.
Dedication, compassion, and reliability.
Willingness to cover on-call shifts and patient visits as needed.
