RN Case Manager – Victoria, TX

Our client is seeking an RN Case Manager to provide field-based hospice care in Victoria, Texas. This role is part of a growing and experienced hospice network known for scaling care teams and expanding into new markets. They’re looking for someone reliable, compassionate, and hands-on, ready to make a difference.

What You’ll Do

• Deliver compassionate hospice care in the field, managing a caseload of patients.

• Coordinate and communicate with the interdisciplinary care team, patients, and families.

• Ensure compliance with hospice standards and maintain accurate, timely documentation.

• Participate in on-call rotation (extra pay provided).

• Complete occasional after-hours visits (paid per visit).

• Play a key role in providing stability and continuity of care to the community.

What You Need

• RN license in Texas (required).

• Hospice experience preferred, but not required (training available for nurses with the right mindset).

• Strong clinical skills and comfort with fieldwork.

• Dedication, compassion, and reliability.

• Willingness to cover on-call shifts and patient visits as needed.
