Title: Senior Coordinator, CHW, LEGHO Program  
Employee Working Location: In-person (Chatham-Kent Branch, ON)
Employment Status: Temporary Full-time (6 months)

In this role, you will:  

  • Coordinate, implement and evaluate the delivery of the HB program.

  • Maintain and update the plan that establishes program goals and objectives for the client.

  • Participate in client discharge planning (hospitals/community support services) 

  • Research and implement strategies to address discharge barriers to support client safe recovery at home.

  • Monitor the efficacy of the planned discharge and time.

  • Approve referrals, complete intakes, and determine internal or external care coordination.

  • Schedule request for service/ transportation and confirm referral acceptance back to the participating hospital. 

  • Follow up with HB clients two (2) weeks after discharge from the program, checking on client wellbeing and assess for any further needs.

  • Complete client satisfaction surveys. 

  • Check IAR for recent assessment following guidelines within established assessment practices

  • Review results of Inter-Rai Screeners to determine need for further assessment and refer to outside agency.

  • Maintain excellent customer service and quality management in service delivery.

  • Assist Management with development and implementation of the annual program work plan to ensure success in meeting Operational targets and ensure the program indicators are met.

  • Support Management in the recruitment, training, coaching, and evaluation of staff and volunteers associated with program delivery.

  • Ensure staff follow safety protocols and policies of the agency and stakeholders.

  • Assist with preparing and monitoring program specific budgets, complete program statistical reports and assist with preparing progress reports.

  • Monitor program operational needs and expenses

  • Seek approval from Management for discharge expenses over $1,000.

  • Program monitoring, evaluating, and reporting.        

  • Provide information/data reports to management as agreed upon or required.

  • Carry out transitions when short staffed.

  • Liaise with hospitals, community agencies and organizations regarding related services and initiatives to foster mutual understanding, coordination, and partnership which will enhance the client experience.

  • Develop and maintain a positive relationship with related hospitals, organizations, and community partners/stakeholders.

  • Participate on external committees relating to mutual client needs.

  • Attend program and external meetings as required.

  • Recognize and develop strategic partnerships to enhance programs and services.

  • Act as a spokesperson for the HB Program.  


 
What we are looking for:  

  • The minimum qualifications for this position are one to three year’s community college diploma in Gerontology or related and between one to three years job related experience or an equivalent combination of education and experience.  Good communication/presentation skills, leadership abilities, and experience working with clients and volunteers is essential.

  • Standard First Aid and CPR-C  

  • Gerontology and/or Social Services training

  • Knowledge of community support services  

  • Professional documenting 

  • Interpersonal skills typically gained through daily living skills    

  • Supervisory or project management experience, including an ability to lead, motivate, and evaluate

  • Ability to develop, implement, and evaluate program plans

  • Public speaking and presentation skills

  • Excellent skills in risk, crisis, and conflict management

  • Strong customer service skills

  • Exceptional telephone skills and interpersonal skills

  • Scheduling experience as asset

  • Ability to work independently, prioritize, multi-task, and achieve successful results

  • Knowledge of medical terminology and documentation is an asset

  • Computer proficiency in Word, Excel, Power Point, Outlook  and databases

  • Fluency in English is required, French is an asset.

  • Clean driver’s abstract, valid drivers license and access to a reliable vehicle.
