As an PH Health PrEP Navigator within the HIV/PrEP Promotion program. At the District Two, Georgia Department of Public Healthyou will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways. District 2 Public Health serves 13 counties in northeast Georgia, including Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White counties. One of the fastest growing regions in the state, District 2 includes rural and urban communities with diverse public health challenges.

Position Duties

Health Navigator:
Under broad supervision by the HIV/PrEP Coordinator & or HIV/PrEP Supervisor, performs a wide range of office administration duties for assigned functions or program areas, duties may be complex in nature and may involve access to confidential information. May research, promotes, maintains, and improves individual and community health by assisting individuals and communities to adopt healthy behaviors in preventing the spread of HIV. Encourage healthy lifestyles, policies and environments within the communities, investigate, and/or resolve program data or issues. Drafts documentation, reports, or presentations. Represents program or unit, provides information and assistance to internal and external customers.
Duties will include, but not limited too within District Two:

  • This position will ensure the development & expansion of the PrEP Program through patient-centered navigation & adherence counseling program.

  • This position will directly work with potential, active, and past PrEP patients in the community, as well as encouraging access to PrEP/HIV Services throughout the District’s communities.

  • Geared towards those at-risk for HIV and those identified in high-risk behaviors or communities.

  • Provide resources and services advisement for those who experience hardships, concerns, disparities and inequalities (although, not limited).

  • This position will provide HIV testing, HIV counseling, HIV Prevention education, PrEP Services education, and Condom Distribution at outreach events within the district.

  • Will be required to complete HIV Testing & Counseling Certification from the State, within 90-days of employment.

  • Will be responsible for filling/submitting HIV Forms for those tested at outreach events.

  • Lifting, stooping, bending, sitting, standing, turning, and flexibility will vary throughout events.

  • Assisting linkage and marketing, when needed with the HIV/PrEP Manager

  • The PrEP Navigator will work with the HIV/PrEP Team to increase HIV/PrEP Services awareness, through marketing/advertisement, communications, presentations, and education/training throughout the district.

  • The PrEP Navigator would be mostly traveling, but infrequent/occasional work-from-home opportunities may arise.

  • The PrEP Navigator will provide condom distribution to our active and potential contracted communities, within the district.

  • The PrEP Navigator will provide communications to patients, healthcare providers, and others to assist the patient with scheduling STD, HIV, and/or PrEP Services – to assist with directing the patient to linkage to care.

  • Including appointment reminders, assistance with insurance approvals, un-insured assistance program forms, and other needs to ensure patient adherence to services and medications.

Minimum Qualifications

Health Navigator:

  • High School Diploma or equivalent.

  • Must be able to lift/pull/push up to 50 lbs, with assistance.

  • Must have reliable mode of transportation


Preferred Qualifications:
  • Utilizes critical thinking skills, sound judgement while engaging with members of the community 
  • Strong communication skills-public speaking, training, facilitating, & writing or coaching
  • Demonstrates the ability to exhibit empathy and understanding to fears, concerns, and hesitancy of individuals 
  • Possesses excellent time management, organizational and communication skills while working both independently & collaboratively
  • Demonstrates proficiency with computers & programs (word, PowerPoint, Excel, and Microsoft applications).
  • Demonstrate knowledge and competence in the use of software: Outlook emails, Word/Excel/PowerPoint, SharePoint/OneDrive, Adobe/Canvas. 
  • Exhibits professional conduct, positive attitude, shows up for work on time and ready to get the job done.
  • Possesses ability to multi-task and be flexible as a liaison/advocate with patients & all relevant professionals through the PrEP program. 
  • Ability to act as a liaison/advocate with patients and all relevant professionals through the PrEP program 
  • Ability to work independently, and collaboratively with a team. 
  • PrEP or HIV knowledge. 
  • Being bilingual is a plus
