The Mobile Crisis Team Leader provides oversight and management of the Mobile Crisis Team (MCT). The Team Leader coordinates administrative activities and functions of the program. The Team Leader responds when community partners request assistance to aid children, families and adults who are experiencing crisis symptoms that put them at risk for hospitalization or other higher levels of care. This position ensures a multi-disciplinary crisis response, collaborating with internal and external partners to respond holistically to crises brought on by mental health, substance use and/or intellectual and developmental disabilities. The Team Leader promotes crisis de-escalation and creative linkages to services that promote stabilization and diversion from higher levels of care. The Team Leader will perform a range of clinical services related to the assessment, care, and treatment of individuals experiencing crisis, in the school, home, community, or in the outpatient clinic setting. The Team Leader will provide clinical leadership and consultation to the MCT as needed and is a positive team player that promotes a culture of inclusion and participation.

Required Education and Experience

  1. Master’s degree in Marriage and Family Therapy, Professional Counseling, Psychology, Social Work or closely related field
  2. License as required by Kentucky professional boards and billing sources
  3. Valid driver’s license, reliable operating transportation, and proof of current auto insurance

Preferred Education and Experience

  1. Qualified Mental Health Professional
  2. Experience working with children, families and adults who experience complex emotional and behavioral difficulties which may present in crisis
  3. One year of administrative and supervisory experience

Essential Functions

       Administrative and Clinical Supervision

  1. Manage and supervise the MCT, ensuring adequate 24/7/365 coverage
  2. Serve as a crisis consultant, providing sound clinical guidance to MCT, organization, and community in real-time
  3. Manage personnel requirements, serve as hiring manager and provide billing supervision as needed
  4. Serve as liaison for Emergency Services within New Vista and in the community
  5. Attend required state licensing board trainings and maintain and advance relevant clinical licensure
  6. Ensure compliance with relevant contract requirements and deliverables, including a 24/7 mobile and telehealth response across the region within 3-hours of request
  7. Ensure MCT compliance with New Vista policy, procedures and required trainings

       Clinical Crisis Services

  1. Assess crisis situations to determine appropriate interventions, partnering with SUD and IDD teams as relevant
  2. Assess and respond appropriately to crisis situations through risk assessment, management, crisis prevention, intervention and de-escalation
  3. As needed, hold a short-term caseload focusing on warm handoffs and rapid connections to ongoing care
  4. Coordinate referrals and transitions into robust services that may include behavioral health outpatient, residential and specialty services (e.g. ACT, TRPs, etc.), primary medical care as well as social determinant needs

Collaboration and Communication

  1. Ensure MCT responds promptly to crisis events and collaboration requests for crisis episodes of care
  2. Collaborate and communicate effectively with the MCT, New Vista staff and community partners, navigating the balance of response and information sharing as it pertains to the specific crisis and who is involved
  3. Positively represent New Vista and the Emergency Services Team at staff and community meetings, fostering internal and external connections
  4. Collaborate with Emergency Services, New Vista staff, and/or community partners to coordinate care that holistically addresses the individual(s) needs and may include outpatient, out-of-home placements, residential treatment and social determinant services (e.g. housing, employment and benefits supports, etc.)

Data Tracking and Documentation

  1. Maintain accurate, thorough records and submit documentation within required timeframes
  2. Track and provide data elements regarding hospitalization episodes, identified quality measures, and other relevant information about client care and program management as requested
  3. Use data to make staffing decisions, demonstrate effectiveness, identify and address gaps to crisis services and creatively highlight the critical role crisis services play in linkages to care, better outcomes and hospital diversion

 

Criss Outreach Specific Functions

  1. Track and provide data elements regarding hospitalization episodes and other relevant information about client care as requested
  2. Respond promptly to requests regarding any crisis episodes of care
  3. Attend/successfully complete all mandatory New Vista Board trainings
  4. Attend required state licensing board trainings and CEUS
  5. Respond promptly to requests
  6. Facilitate out-of-home placement and/or higher level of care as needed
  7. Collaborate with Emergency Services Teams, other New Vista team members, and/or community partners in facilitating out-of-home placements. As needed, attend internal meetings for hospitalization placements
