We are seeking a Speech-Language Pathologist (SLP) to join our Therapy Department. Why We’ll Love You:

Responsibilities:
  • Performs all direct resident care and services in accordance with Federal, State, local, and Community First Solution’s standards, policies, procedures, and guidelines; the “Client’s Bill of Rights”, and the “Principles Guiding our Actions”.
  • Evaluate and diagnose communication difficulties in residents, including presence, type, and degree of impairment.
  • Determine extent and type of treatment for assigned residents and treat communication disorders and delays of residents.
  • Treat residents individually and in groups in accordance with the needs identified in the treatment plan for the resident’s care.
  • Document client care in a timely manner, to include: assessments, plans of care, progress notes, discharge summaries, incident reports, billing information and other documentation in accordance with policies and procedures.
  • Develop and periodically revise individual plans of care, including long-term care goals and short-term objectives.
  • Train parents and families, staff members, and other healthcare professionals on the resident’s therapeutic process and care.
  • Attend all scheduled staff meetings and participate in the Quality Improvement Process program when requested.
  • Adhere to infection control policies, professional therapeutic services, and Community First Solution’s standards, policies, procedures, and guidelines.
  • Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.

  • All other duties as assigned and/or appropriate to the position.
Qualifications:

Education:  Master’s degree from an accredited Speech and Language Pathology program.

Licenses or other required certifications: Ohio License as a Speech-Language Pathologist. CPR certification.

Experience: At least two years’ experience as a Speech-Language Pathologist.  Experience with geriatrics and long-term health care setting preferred.
