We are seeking a Therapy Services Aide to provide transportation for residents to therapy appointments and offer administrative support to our therapy team. In this role, you’ll help ensure smooth operations, promote resident safety, and assist our therapists in delivering high-quality care. Responsibilities:
- Transports patients/residents as scheduled to therapy appointments
- Uses equipment properly and accurately when providing transportation (e.g., wheelchair, gait belt, Hoyer lift, etc.).
- Reports any change of condition, incidents, accidents and/or abuse to the treating therapist and/or Therapy Manager immediately.
- Communicates with the therapy staff on each assigned patient/resident especially noting those at risk for falls
- Responsible for clerical and administrative support such as computer work, copying, filing, and faxing orders and papers as directed by the Program Manager.
- Participates with team members to meet the psychosocial needs of patients/ residents.
- Maintains a focus on patient/resident protection and safety when providing care.
- Maintains the cleanliness, general maintenance and organization of the gym and rehab equipment.
- Must have the ability to complete assignments in an accurate and timely manner

- All other duties as assigned and/or appropriate to the position.
Education: High School graduate or equivalent.
Licenses or other required certifications: State of Ohio certification for nursing assistant. CPR certification.
Experience: Experience in provision of patient/resident transportation and care preferred. Demonstrates good verbal and written communication skills.
Specialized knowledge, skills, or abilities: Knowledge of aspects of long-term care preferred.
