Community Health Worker ( Women’s Health)

Work Schedule

8 a.m. to 5:30 pm Monday- Thursday.  8 a.m. to 12:00 pm on Friday. 

However, adjustments in schedule or additional hours may be required to complete duties.

 

POSITION DESCRIPTION:

The Women’s Health Community Health Worker is part of the maternal health care team charged with creating and integrating patients into health programs to assist with targeted health outcomes. The Women’s Health Community Health Worker will develop, utilize, and integrate community program and resources as part of the development of an effective program. The Women’s Health Community Health Worker directs and is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.

 

SPECIFIC DUTIES AND RESPONSIBILITIES:

  1. Promote customer-driven excellence as a driver of organizational services and goals.
  2. Meet with patients, individuals, and/or community organizations as needed to support the patient and program development.
  3. Ensure efficient tracking and documentation of all activities and interaction with each patient in a timely manner.
  4. Participate in community events and outreach efforts to raise awareness and develop relationships
  5. Assist with coordination of care with the clinical team, other Community Health Workers, and/or community groups based on the needs of the patient.
  6. Participate in home visits to engage pregnant and postpartum women, providing education related to pregnancy, nutrition, and breast feeding; and SDOH assessment.
  7. Maintain contact using variety of mediums to check on their status, progress, barriers, needs and assist with coordination of care including home visits.
  1. Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  1. Performs all job duties in compliance with MHSI Safety and Infection Control policies.
  2. Assists in developing reports to meet organizational internal and external requirements. 
  3. Assists patients with insurance outreach and enrollment 
  4. Maintain inventory of maternal health supplies as related to the Women’s Health CHW program, tracking supply usage to ensure timely replenishment.
  5. Coordinate with internal staff and external partners to procure and distribute supplies

 

REPORTING RELATIONSHIPS:

The Women’s Health Community Health Worker reports to, is supervised by the Director of Care Management.

 

EVALUATION:

The evaluation of work performance will be ongoing and will be completed by the Director of Care Management. Specific duties and responsibilities of this position description plus employee ability to interact constructively with clinical and administrative staff and general working behavior. Formal evaluation will be conducted annually but may be initiated at other times by the Director of Care Management.

QUALIFICATIONS:

  1. A high school diploma or equivalent required;  Associate’s degree or higher in health or human services preferred
  2. Experience in the substance abuse and/or mental health field preferred.
  3. Functional knowledge of Microsoft Word, Excel, Outlook, and EMR Systems
  4. Additional Qualifications: Possession of a valid Arkansas Driver’s license is required. Independent travel is required. Position will be up to 80% travel within the MHSI service area and occasional meeting outside the area.
  5. The ability to work in a constant state of alertness and in a safe manner.

This is a FLSA Non-Exempt position.
