Reports To: Director of Programs

Job Title:  Quality Assurance Coordinator 

Reports To: Director of  Programs           

POSITION SUMMARY:  Reporting to the Director of Programs, the Quality Assurance Coordinator oversees a client caseload of a mix of Community Living Support, Supported Living, and Residential clients.  This position ensures Rise & Shine’s quality assurance standards and procedures are met by reviewing Oregon Administrative Rules and ensuring policies and program(s) are in alignment.

ESSENTIAL FUNCTIONS:

  • Fosters positive working relationships between staff and leadership, clients, and community parties.
  • Coordinates with agency nurse and/or Behavioral Specialist, and other professionals so client needs are being met on a timely basis.  Organize and assist in the development plans of clients.
  • Ensures that policies, procedures, and necessary forms are updated and implemented as required by additions or modifications to OARs. 
  • Updates policies, procedures and make recommendations for the overall development of the program(s).
  • Assists in the training and development of staff.   Partner with trainers to ensure training folders are complete.
  • Partners with the Behavior Professionals and Program Managers to ensure client behavioral support and individual support plans are being followed.   
  • Ensures licensing requirements are being met and are operated at the highest level of care possible by supporting sites and managers with resolving QA reviews and licensing Plans of Improvement. 
  • Provides an administrative review of General Event Reports (GERs) within 3 business days, communicating trends, concerns, and other issues of concern to Directors and Executives. 
  • Responsible for updating information in Therap, creating, and utilizing reports.
  • Provides feedback and participate in performance reviews of staff.
  • Ability to work any shift in coverage of direct support staff and cover on-call/ minimum one week/weekend each month. 
  • Ensure OSHA compliance at the program site. 
  • Other duties as assigned.  

SKILLS & ABILITIES 

  • Experience writing, reviewing, and implementing policies and procedures.  
  • Ability to review and critique Behavioral Support Plans (BSPs) and Nursing Care Plans. 
  • Knowledgeable of Oregon Administrative Rules (OARs), updates, and licensing. 
  • Must have and demonstrate a positive, solutions–oriented mindset, and a positive attitude.
  • Must have a valid Oregon driver’s license and be insurable by the company provider. 
  • Must pass Oregon criminal background screening. 
  • Must be able to lift up to 25 lbs., bend, and/or squat. 
  • Must be able to walk up and down stairs.
  • Must be able to perform the job with or without a reasonable accommodation.

MINIMUM QUALIFICATIONS: 

Education/Experience:

  • 2 years of experience in a residential, educational, or vocational setting serving people with disabilities or the elderly.
  • Bachelor’s degree in social work, special education, psychology, or a related field; Or equivalent combination of education and experience.  

Computer Skills: 

  • Proficient with Microsoft Office Suite or similar software, Therap experience preferred.

Physical Demands: 

  • Must be able to lift up to 25 lbs., bend, and/or squat.
  • Must be able to walk up and down stairs.
  • Must be able to sit/stand for long periods of time.
  • Must be able to perform the job with or without a reasonable accommodation.
