Job Summary: Provides and coordinates high-quality, patient and family centered, trauma-informed care to achieve mutually established patient goals with a team-based approach consistent with the mission and vision of PCC. LPN responsibilities and duties are overseen by the Registered Nurse Team Lead.
Essential Duties and Responsibilities:
- Provides individualized, culturally sensitive and trauma-informed nursing care to medically complex and underserved patient population with multiple co-morbidities including behavioral health and chemical dependency related diagnoses.
- Triages emergent, urgent and routine patient phone calls, messages and walk-in patients both PCC and non-PCC using evidence-based triage guidelines; provides appropriate direction to patients and consultation/report to providers/EMS/911 based on critical thinking and nursing process.
- Reviews, triages and relays to provider all lab/procedure results ordered by multi-specialty providers; and incoming from external specialties based on established emergent, urgent and routine criteria and nursing assessment.
- Maintains/updates knowledge in multiple content areas including family practice, pediatrics, OB/GYN, midwifery, cardiology, behavior health, chemical dependency, school-based care and care of those with housing insecurity.
- In response to acute lab results assessment, initiates PCC Standing Orders to treat patients over the phone and/or in office. Notifies patient by telephone or certified mail of any abnormal test results as directed by provider, provide and assess understanding of results-related counseling and education and next recommended steps for patients with multiple comorbidities. Outreaches to patients lost to follow-up regarding important test results.
- Assesses and documents past medical history, family medical history, past treatments, medication reconciliation, social and sexual history and other screenings for new and return patients when indicated.
- Obtains and assesses vital signs and performs routine functions to prepare patients for examinations or procedures during nurse-driven visit and/or provider visit, as indicated.
- Assists providers with in-office treatments and procedures; may perform laboratory and diagnostic tests ordered by the provider, when indicated.
- Directs and supports clinical and non-clinical staff in need of assistance to maintain patient flow.
- Leads (and identifies need for) nurse-driven visits for but not limited to: blood pressure check; diabetic teaching (device and concepts); insulin titration; wound care; medication reconciliation and adherence support; urinary symptom screening; point of care testing, including pregnancy test and INR monitoring; STI screening, testing, treatment, counseling, education. Consults with and collaborates with onsite provider per nursing assessment and PCC policy.
- Administers oral and long-acting injection medication/treatments as indicated in PCC Standing Orders and as prescribed by multi-specialty providers per PCC policies, procedures and workflows.
- Counsels, orders, administers and monitors for side effects of COVID-19 vaccines; complete rapid/PCR COVID-19 patient and employee testing and provide detailed/individualized results notification/counseling; ensures COVID-19 test result reporting for patient/employee; prepares test results notification letters as indicated by PCC policy/workflow.
- Tracks and ensures ongoing patient adherence to Coumadin therapy and indicated PT/INR monitoring.
- Ensures notification and treatment of all patients with positive STI results; ensure IDPH notified of disease diagnosis and treatment for reportable infectious diseases.
- Assesses all incoming chronic, specialty and acute medication refill requests and facilitates renewals based on established PCC workflows, consulting with provider as needed/indicated. Initiates and facilitates medication prior authorization request process with provider.
- Assesses the patients condition and learning needs in regards to health care and provides targeted individualized teaching/education plans for patients with multiple comorbidities based on nursing assessment and provider direction. Utilizes teach back method to provide necessary patient education.
- Accurately updates and maintains medical record; documents and reports to provider pertinent information regarding patients condition and care plan according to PCC policy and procedures.
- Keeps providers informed of changes in assignments or patient flow that affect working conditions.
- Cross-covers other clinic sites to ensure continuity of high-quality nursing care across the organization, when indicated.
- Learns and implements new evidence-based clinical practices into existing nursing practice.
- Assists Nurse Team Lead and other clinical staff with the orientation of new staff. Functions as a resource for all clinical and non-clinical support staff.
- Assesses educational needs of clinical staff, provides additional training as needed. Initiates and pursues own plan for continued professional growth. Attends evidence-based educational sessions from external and internal content experts.
- Adheres to PCC customer service guidelines and protocols. Maintains patient confidentiality.
- Complies with PCC policies, procedures, and safety- and equipment-related events and practices.
- Drives continual quality improvement through participation in organizational quality improvement efforts that may include but are not limited to patient satisfaction, clinical and operational quality improvement initiatives.
- Ensures work area is stocked with necessary supplies/materials. Maintains a neat, orderly clinic work area.
- Attends nurse and other staff meetings as scheduled.
- Performs other duties as assigned.
Qualifications
Experience/Training:
- Graduate of an accredited school of nursing.
- Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.
- 2 3 years clinical experience preferred.
- 2 - 3 years EMR experience preferred.
- Proficient in MS Office Business Applications to include: Outlook, Word, PowerPoint, and Excel.
- Certifications/Licenses:
- Current and valid State of Illinois nurses license.
- Current and valid CPR card.
- Physical Demands:
- Must be able to remain in a stationary position 50% of the time.
- Must be able to move around the clinic site 50% of the time.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Ability to move as needed to perform necessary nursing skills such as wound care, injections, and other procedures.
- Must be able to transport from one site to another.
- Must be able to cover other shifts and sites as necessary.
