The SLP acts as a resource to other disciplines, such as Nursing, Medicine and Nutrition and provides education to team members, residents and family members related to communication, swallowing and cognitive deficits Description of duties for a Speech Pathologist include, but are not limited to: %26bull; Performs admission, annual, quarterly and significant change assessments of swallow within designated timeframes, makes clinically sound recommendations and determines if further evaluation and/or treatment is warranted. %26bull; Evaluates residents and ADC registrants as prescribed in an MD order and establishes a treatment plan with realistic goals that are reasonable and attainable. %26bull; Administers and interprets specialized testing as required. Communicates and incorporates results from testing and appropriate interventions in the CCP with input from the IDT. %26bull; Reviews hospital reports, tests and recommendations from prior and ongoing clinical investigations and integrates information into the clinical care plan. %26bull; Carries out treatment plans with clinically sound interventions to progress towards specific goals. %26bull; Documents and communicates recommendations and protocols for unit-based interventions to nursing staff and other involved disciplines. %26bull; Communicates diet recommendations to the MD for appropriate diet orders and to the Nutritionist for coordination of diet consistency. %26bull; Facilitates obtaining communication devices through outside sources, such as the VA or other vendors, to maximize each resident%26rsquo;s/registrant%26rsquo;s ability to communicate. %26bull; Completes all necessary documentation of Therapy programs, such as Evaluations, Progress Notes, Daily Notes, care plans and the MDS. Follows guidelines in all documentation to meet regulatory requirements for reimbursement and accurately communicates the clinical care provided. %26bull; Provides education to employees, family members and orientees in safe swallowing techniques. %26bull; Mentors graduate clinicians as per federal, state, American Speech-Language Hearing Association (ASHA), and facility guidelines. %26bull; Provides undergraduate observation and clinical opportunity for learning through ongoing program with Stony Brook Speech Pathology program. %26bull; Performs daily billing, and checks MD orders. Qualifications Required Qualifications: Masters Degree in Speech and Language Pathology, Current and valid New York State license in Speech-Language Pathology, American Speech-Language Hearing Association (ASHA) certification in Speech-Language Pathology (CCC%26mdash;Clinical Certificate of Competency), A minimum of two years of experience in Adult therapy with experience in both swallowing and communication. Preferred Qualifications: One year or more year experience in a skilled nursing facility, FEES competency, Vital Stim competency.
