Position Summary: Serves as an advocate and resource to The Meadows and Brickstone Residents. The Elder Advocate will act as a liaison between the Resident, their family members/caregivers and various elder support systems throughout the St. John’s and Greater Rochester community. Elder Advocate will Work to identify opportunities which promote quality of life for the Residents as well as striving to identify options to sustain the highest level of independence that is safe for each Resident. Provides direct supervision to Part -Time Elder Advocate.  

 

Qualifications & Education Required:  

  • A Master’s degree in Social Work from an accredited college or university in an approved course of study, plus  one year of related work experience; or
  • A Bachelor’s degree in social work from an accredited college or university in an approved course of study, plus three years of related experience.                                                      
  • A valid driver’s license.

Position Responsibilities:

  • Support Eden philosophy.
  • Assist and support Residents through life transitions by assessing needs and assisting Residents, their families and responsible parties by identifying supportive options to promote a secure and safe living environment.
  • Acts as a referral source between Residents, their families and responsible parties and other disciplines and services throughout St. John’s community.
  • Remain knowledgeable on long term care insurance, government programs, other health insurance programs, financial management services and public benefits. Be able to explain the eligibility requirements and enrollment process for these benefits to Residents, their families and responsible parties.
  • Welcome and introduce the Elder Advocate role to all new Residents within 30 days of move in.
  • Collaborate with Marketing team and Administration when Residents make transitions to other levels of care.
  • Communicates and works with Administration regarding complex Resident situations or patterns of need in the Meadows and Brickstone communities.
  • Assist Residents, their families and responsible parties with understanding the process for moving to a higher level of care.
  • Assist residents in obtaining durable medical equipment.
  • Create, update and maintain lists of St. John’s specific services available to Meadows and Brickstone Residents as well as lists of Rochester community based services of interest.
  • Actively participate in GRAPE.
  • Make referrals to Monroe County Adult Protective or the Mobile Crisis team as needed.
  • Communicate with community health care providers. Address any concerns with Program Administrators.
  • Attend and participate in community chats.
  • Provides and oversees educational presentations and workshops relevant to the well-being of residents and needs of the community.
  • Support St. John’s mission, vision and values.
  • Will be required to work flexible hours.
  • Provides guidance and supervision of the part time Elder Advocate position. 
  • Oversee Interview and hiring process for part time Elder Advocate Position.
  • Communicate all administration policy and procedures and their changes to department staff.
  • Perform all other tasks as required by management

 

Physical Requirements:
