The Therapy Care Coordinator (“TCC”) must be committed to patient customer satisfaction. Supports physical and occupational therapy by their primary role being to oversee and coordinate all therapy orders/scripts. The TCC is a vital role in helping the patient find the best option for attending therapy once their physician has deemed it necessary.
This position will be tasked with increasing patient capture rate and will require daily, weekly, monthly dialogue and analytical discussions about clinic metrics.
This individual will be decisive, self-driven, and dynamic; also, willing to be mentored, trained, and developed to achieve high performance and personal satisfaction.
This position will report directly to the Clinic Director.
Key Responsibilities:
- Greet any patient with a therapy order/script and escort them to the therapy department to tour the facilities and introduce to the therapy staff.
- Schedule the patient for therapy. If the patient is unable to attend therapy at our clinic (inbound), the TCC will help find a location that is acceptable to the patient and get the appointment scheduled (outbound).
- Will coordinate with the Rehab department to assist with any duties that the TCC can perform for the department while at the TCC station; though not to interfere with patient services.
- Administration of the patient therapy script que and pending insurance verifications and
- authorizations.
- Complete patient reminder calls, reschedule patient appointments, and maintain patient call logs.
- Verify insurance benefits and authorizations
- Update and maintain all TCC systems and spreadsheets as outlined in SOP’s.
- Actively assist in the administration of FOTO and FOTO systems.
- Maintains patient confidence and protects operations by keeping information confidential.
- Adheres to policies and procedures, federal, state, and local requirements.
- Enhances physical and occupational therapy department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
- Updates job knowledge by participating in education opportunities; reading publications, maintaining personal networks; participating in professional organizations.
For inbound:
- Patients will receive a Therapy Welcome packet that contains information about the clinic, clinicians, and what to expect.
- Patients are to be scheduled within 48 hours of the date of the script.
- TCC will log the patient script on the inbound excel spreadsheet and fill out the appropriate data.
- TCC will check to ensure that all POCs, re-evals, progress notes, and DC summaries are sent to the physician before to the date of the follow-up visit with the physician or when the particular note is generated (whichever occurs first).
- Will track the inbound capture rate on a daily, weekly, monthly, and yearly basis
For outbound:
- Patients are scheduled within 48 hours of the date of the script.
- TCC will fax or email the therapy order and appropriate surgical protocol to the outbound therapy clinic where applicable.
- TCC will log the script on the outbound excel spreadsheet and fill out the appropriate data.
- TCC will check to ensure that all POCs, re-evals, progress notes, and DC summaries are sent to the physician before the date of the follow-up visit with the physician or when the particular note is generated (whichever occurs first).
