Role Overview
We’re hiring PMHNP-BC clinicians to provide on-site and telehealth behavioral health services for residents in Assisted Living/RCFE communities across Stockton. Immediate need: ~300 eligible residents are ready to be seen across our Stockton assisted living communities, providing consistent, predictable volume.
What You’ll Do
- Psych evals & follow-ups; med initiation/titration/monitoring
- Coordinate with PCPs, facilities, families/POAs, and our concierge team
- Same-day documentation in EMR; regulatory/CMS compliance
- Staff education (behavior plans, safety, de-escalation basics)
Requirements
Qualifications
- PMHNP-BC; active CA NP & RN licenses; active furnishing, NPI, DEA, BLS
- Senior living/ALF/RCFE experience preferred; EMR proficiency
Benefits
Compensation (CA pay transparency)
Annual compensation range: $175,000–$210,000 (based on experience, schedule, and patient volume)
Additional earning opportunities available based on coverage and panel needs
W-2 with benefits available; mileage reimbursed per policy
Final terms outlined in the employment agreement
Schedule & Setting
Part-time; weekday blocks preferred. Clustered Stockton routes with optional tele blocks.
