At the core of our program is a transformative approach embedding a Resident Advocacy Director (RAD) into our communities.
We’re seeking a Regional Care Coordination Specialist who will serve as the operational leader over a region of communities. This is a high-impact opportunity to be a foundational leader in a new standard of care across the Navion portfolio while engaging and supporting prospective residents throughout the region. You will play a key role in building the operational backbone for our future clinical services, creating a legacy of excellence and innovation.

This role will serve several different areas within the North Central North Carolina region.
Requirements
- Strong clinical background with proven experience in care coordination, case management, or a direct care role within a healthcare setting.
- Experience in geriatric care, particularly with dementia and cognitive decline
- An understanding of clinical workflows, resident assessments, and care plan development.
- Highly organized and self-motivated, with the ability to manage a caseload of residents across multiple communities.
- Exceptional ability to build rapport and communicate effectively with residents, families, and community team members.
Preferred Skills & Attributes
- Active Registered Nurse (RN), Licensed Practical Nurse (LPN), Physical Therapist, Occupational Therapist, Physical Therapist Assistant, Certified Occupational Therapist, or a strong operational leader with a background in multi-site healthcare operations.
- Familiarity with senior living environments (Independent Living, Assisted Living, Memory Care) and the roles of key team members, such as the Director of Clinical Services (DCS) and Resident Care Coordinator (RCC).
- Experience managing or implementing CMS care coordination programs.
- Experience creating clinical training materials or standard operating procedures.
Key Responsibilities
In this role you will:
- Support successful implementation and integration of Resident Advocacy Programs within our communities. Your leadership will include supervision of the embedded Resident Advocacy Director including hiring, training, successful program implementation, and measuring outcomes.
- Serve as the go-to authority on all care management initiatives delivered through the Resident Advocacy Program, including GUIDE, Principal Care Management, Chronic Care Management, and hospice administration.
- Ensure all program activities and documentation meet or exceed Centers for Medicare & Medicaid Services (CMS) standards for quality and compliance.
- Coordinate with community staff, the RAD, and third-party care providers within the community to assure a “one true north” of care management delivering high quality care and quality of life to residents.
- Coordinate with regional and community leadership of Navion to assure complete integration within the community.
- Support education of residents, families, and community staff to the value and full utilization of care management provided through the Resident Advocacy Program.
- Build referral relationships with local providers, primary care practices, hospitals, skilled nursing/rehab facilities, home health, hospice, palliative care groups, and other community organizations supporting older adults, educating them on RAP services and identifying patients who may benefit from enrollment.
- Enroll local qualified residents into the GUIDE Program.
- Manage the ongoing, monthly care coordination requirements for all enrolled residents in your region, including regular touchpoints with residents and their families.
- Continue to oversee the success of the program with audits, clinical metrics, and enrollment.
