The Transport Driver / Certified Nursing Assistant (CNA) plays a dual role in supporting the health, safety, and well-being of residents. This position is responsible for providing compassionate, hands-on care to residents while also ensuring safe and courteous transportation to medical appointments, errands, and facility-related outings. The ideal candidate demonstrates strong interpersonal skills, attention to detail, and a commitment to enhancing the quality of life for residents through both caregiving and mobility support.
Position Responsibilities:
This role combines responsibilities in both resident transportation and direct caregiving. The employee is expected to perform duties with professionalism, safety, and compassion, ensuring the well-being and dignity of all residents.
Essential Functions
- Provide safe, courteous, and timely transportation for skilled nursing residents to medical appointments, facility events, and errands.
- Ensure resident safety during boarding, transport, and unloading, including those who are ambulatory or use wheelchairs.
- Assist residents with packages or personal items as needed.
- Maintain communication with facility staff via cell phone regarding resident pick-up and drop-off times.
- Accurately log transportation details, including:
- Departure and return times
- Date and purpose of transport
- Resident name and destination
- Notification of nursing staff upon resident return and delivery of any accompanying medical documentation
- Park the vehicle in the designated area after use.
- Maintain the cleanliness and safety of the transport vehicle after each use.
- Perform routine vehicle checks and report maintenance needs to the Maintenance Director.
- Keep daily mileage and maintenance logs.
- Follow all traffic laws and facility transportation protocols.
- Exhibit safe driving and work practices at all times.
- Assist residents with activities of daily living (ADLs), including:
- Bathing, dressing, grooming, toileting, and mobility
- Changing bed linens, repositioning, and walking/exercise routines
- Feeding residents and preparing them for meals/snacks
- Review care assignments and provide care as outlined in the resident’s plan of care under the direction of a licensed nurse.
- Maintain accurate and timely documentation of care provided.
- Promote a respectful, dignified, and supportive environment for all residents.
- Maintain professional relationships with residents, visitors, staff, and regulatory personnel.
- Understand and comply with all resident rights and confidentiality regulations.
- Participate in orientation, training, in-service education, and staff meetings.
- Assist in resident recognition programs and other facility activities as needed.
- Follow all safety and sanitation protocols and report hazards or violations promptly.
- Perform additional duties as assigned by the supervisor.
Qualifications
Qualifications:
- High school diploma or GED.
- Valid driver’s license in the state of Pennsylvania with a suitable driving record.
- Minimum age of 21 years or older.
- Current CNA certification in the state of Pennsylvania, preferred.
- If no CNA certification, we will pay to send to CNA Class.
- CPR and First Aid certification preferred.
- Prior experience in transportation within a healthcare setting, long-term care, or assisted living is preferred.
- Ability to safely operate passenger vans or wheelchair-accessible vehicles.
- Strong knowledge of local traffic laws and safe driving practices.
- Ability to assist residents with mobility needs during boarding and exiting.
- Ability to lift, move, and assist residents using proper body mechanics.
- Strong understanding of infection control, patient safety, and resident rights.
- Compassionate, patient, and respectful demeanor.
- Ability to document care accurately and follow individualized care plan.
