Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation located, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. Position Overview: The goal of the Passport Care Manager is to assess, arrange, coordinate, monitor, evaluate, educate, and advocate for a package of multiple services to meet the consumer’s needs in the community.

 

Job Duties:

  • Conduct comprehensive in-person needs assessment on an ongoing basis and as required. Assessment includes obtaining the consumer’s health history through consumer/caregiver reporting, data collection, medical records, and collaboration with physicians and related health care professionals. Assessment addresses the consumer’s functional, psychosocial, socio-cultural, and resource/financial status in relationship to their strengths, challenges, opportunities, and formal/informal support systems.
  • Establish a care plan to meet the consumer’s health care needs in the community. Care planning includes direct intervention by the care manager and direct service provision from community resources and from the contracted provider network. Care planning addresses specific consumer outcomes. Care planning at the system level includes reporting unmet needs to the appropriate department for strategic planning within the Agency for resource development.
  • Implement and coordinate the delivery of services through the authorization of contracted services, through referrals to community agencies and through the mobilization of consumer’s informal supports.
  • Evaluate and monitor the appropriateness of the care plan in meeting consumer needs based on consumer outcomes and make necessary adjustments to the plan to assure that goals are met.
  • Advocate in the best interest of the consumer through negotiation, collaboration, and communication on issues relating to the goals and objectives of the established care plan. In doing so, the care manager may act as a representative, liaison, mediator, or educator to obtain and maintain the most supportive environment for the consumer.
  • Educate the consumer on issues of health promotion, illness prevention, crisis management, general well-being, and in maneuvering within the health care system.
  • Educate the consumer and service providers regarding the roles and responsibilities of each party according to the level of care management prescribed by the care manager.
  • Perform other relation duties as assigned by the Care Manager Supervisor, Clinical Manager, and/or Director of the Community Services and Supports department.
  • Participate in trainings, supervisory meetings, in-services and continuing education opportunities.
  • Perform other related duties as assigned.
  • All staff have the responsibility and authority to propose new ways to improve quality and to fully participate in quality improvement efforts.
  • This position’s primary duty is to perform work directly related to the successful operation of the Agency. Accordingly, the employee must exercise discretion and independent judgment with the respect to matters of significance.
  • Abide by the professional code of ethics established under the individual licensure.
  • May be asked to support practicum experiences of students from educational institutions. 


Requirements:

  • Current licensure as a social worker by the State of Ohio Counselor and Social Work Board or as a registered nurse by the Ohio Board of Nursing.
  • Two years’ experience in home care, geriatrics, home health care, medical social work, or other substitute as approved by the Ohio Department of Aging.
  • Excellent verbal and written interviewing and assessment skills and knowledge of local resources for older adults.
  • Ability to communicate in a collaborative, effective manner with others and maintain good working relationships.
  • Basic knowledge of Microsoft Office applications.
  • Ability to use and will be trained on the Ohio Department of Aging PIMS documentation program.
  • Ability to use and transport a laptop computer and case, therefore must be able to lift/carry a minimum of twenty (20) pounds.
  • Ability to ambulate enough to access private homes/locations which may not be readily accessible.
  • Must have a car available during working hours, as well as the ability to legally drive a car.
