Job Summary:
The Hospice LPN - Clinical Administrative Support role is responsible for providing both direct nursing care to hospice patients and essential administrative support to the clinical management team.
This role supports overall clinical efficiency by balancing field nursing responsibilities with administrative oversight, ensuring high-quality care delivery, accurate documentation, and smooth communication between field and office operations.
Clinical:
- Provide direct nursing care under established policies, procedures, and the nursing care plan.
- Perform skilled nursing visits, treatments, and procedures with attention to detail, safety, and patient comfort.
- Administer medications, check supplies, reorder as needed, and maintain accurate patient charts.
- Record and report changes in patient condition or behavior promptly.
- Take and document vital signs accurately.
- Participate in patient and family education and support.
- Maintain adaptability to changes in workload or patient needs.
- Communicate patient updates to the Interdisciplinary Team and contribute to care planning.
- Build and maintain respectful, compassionate relationships with patients and families.
- Participate in in-services, staff meetings, and new employee orientation.
- Exercise sound judgment and seek guidance when needed.
- Maintain assigned equipment and report malfunctions.
- Perform duties accurately and independently, including after-hours calls.
- Complete assignments thoroughly and on time while maintaining quality work.
- Perform other related duties as assigned.
Administrative:
- Provide clerical and administrative support to the Director of Clinical Services and Case Managers.
- Assist with scheduling, staffing calendars, and patient visit coordination.
- Prepare reports, correspondence, and meeting materials.
- Review and organize clinical documentation for accuracy and compliance.
- Track admission packets, consents, and coordination forms for timely filing.
- Support communication between field and office staff to ensure efficient care coordination.
- Assist with onboarding, orientation, and training of new clinical staff.
- Attend departmental meetings, prepare materials, and record meeting minutes.
- Relieve management of routine clerical duties to support focus on clinical oversight and team performance.

Qualifications
Required Skills/Abilities:
- Excellent communication, customer service, and organizational skills.
- Attention to detail and ability to meet deadlines in a fast-paced environment.
- Proficiency with EMR systems and Microsoft Office Suite.
- Ability to balance multiple priorities and transition between clinical and office settings.
- Strong problem-solving and time management abilities.
- Professional demeanor when interacting with patients, families, and staff.
Education, Licenses, Certifications and Experience:
- High School Diploma, or GED equivalent.
- Licensed Practical Nurse Certification.
- Valid CPR Certification.
- 1 year of experience in home health, LTCF, or related field nursing preferred.
