Remote Care Coordinator (RCC)
Location: Remote ( Must reside in Georgia)

Part-time. 20 hours per week.
Overview
The Remote Care Coordinator (RCC) is a key member of Ascend Medical’s Chronic Care Management program, responsible for providing consistent, proactive outreach and coordination for patients enrolled in ongoing care plans. The RCC supports Ascend’s integrated approach to Whole Person Care—working across primary care, behavioral health, and restorative health and beyond to ensure that every patient receives comprehensive, coordinated support.
This fully remote, patient-facing role combines strong communication skills, EMR documentation accuracy, and the ability to drive measurable results through consistent patient contact and time logging. The RCC must be able to engage with patients and families in a supportive, empathetic, and culturally competent manner.

Primary Responsibilities
Patient Engagement & Care Management
• Conduct monthly outreach calls to patients under active CCM enrollment, ensuring a minimum of =20 minutes of monthly interaction per patient.
• Document all interactions, care plan updates, and interventions within Athena and ThoroughCare according to program standards.
• Coordinate care between primary care, behavioral health, and ancillary service lines to ensure comprehensive Whole Person Care.
• Support patients in understanding care plans, medication adherence, and next steps in their treatment journey.
Collaboration & Communication
• Partner closely with assigned on-site Care Coordinator (CC) in a pod-based structure.
• Maintain active communication with providers, clinical teams, and Operations Managers regarding patient status and follow-ups.
• Escalate clinical concerns or non-adherence trends to CCs or providers promptly.
Operational Excellence
• Track and log all CCM minutes accurately.
• Participate in bi-weekly huddles to review patient status, performance, and workflow updates.
• Follow company policies regarding documentation standards, compliance, and patient confidentiality (HIPAA).
Qualifications
Required: • 2+ years in healthcare coordination, nursing support, or case management • Experience with electronic health records (EHR/EMR), ideally Athena and/or ThoroughCare • Strong communication, follow-up, and patient education skills • Ability to engage with patients and families in a supportive, empathetic, and culturally competent manner • Comfort with outbound calls, documentation, and multitasking in a remote setting
Preferred: • Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) credential • Experience with chronic care management, transitional care, or population health programs • Familiarity with Medicare billing, CPT 99490/99439
Work Environment & Schedule
• Fully remote position, operating primarily during standard business hours (8:00a–5:00p EST) • Must have reliable internet, quiet workspace, and ability to handle confidential information • Occasional virtual meetings and training with operations and clinical leadership
