Summary: The Family Connects Nurse Home Visitor conducts in-home or telehealth visits for families with newborns in Bexar County, who deliver at CHRISTUS Children’s Hospital. The aim is to assess family needs across 12 domains of well-being and facilitate connections to community agencies for support. This role collaborates with clinical and non-clinical staff, adhering to the evidence-based Family Connects protocol.
Position Responsibilities:
o *Adhere to the Family Connects protocol for in-home and telehealth visits by performing health and physical assessments of mothers and babies.
o *Evaluate family needs in 12 domains of well-being, exercising clinical judgment.
o Build trust with families, develop support plans, and facilitate referrals to community resources.
o *Apply expertise in parent-child interaction, child development, and family health to support parenting and reduce stress.
o *Maintain 8 visit slots per week and conduct home visits regularly, adjusting schedules as needed.
o *Follow up with clients after visits and Post Visit Connection calls and coordinate additional support as needed.
o Support team members and promote a positive work environment.
o Represent and promote the vision and mission of Family Connects in a professional manner.
o *Participate in quality improvement efforts and professional development.
o Perform other duties assigned by the Program Director.
Education and Support:
o Offer educational materials and support to families on topics such as infant care, parenting skills, nutrition, and healthy lifestyle choices.
Documentation:
o *Maintain accurate and up-to-date records of interactions with families, including assessments, referrals, recruitment outcomes and follow-up actions taken in accordance with the organization’s policies and procedures.
Collaboration:
o *Work closely with the Family Connects team, including Nurse Home Visitors, program coordinators, and other support staff members, to ensure seamless coordination of services and effective communication. Focus on solutions and teamwork.
o Communicate effectively with families, colleagues, and stakeholders.
Community Outreach:
o Participate in community outreach activities to promote awareness of Family Connects services and engage with potential clients.
o *Participate in and contribute to the organization and execution of events aimed at fostering the growth and advancement of program services and partnerships.
o Conduct client recruitment as necessary for the program. Additionally, assist with data collection and reporting on recruitment outcomes.
Professional Development:
o *Stay informed about best practices in nursing, maternal and newborn health, community health, home visiting programs, and relevant resources through ongoing training and professional development opportunities.
o *Attend meetings, trainings and completes assigned professional development in a timely manner
Program Quality Improvement and Administration
o Adherence to the Code of Conduct and the Faith and Moral is mandatory.
o Must be sensitive to the service population’s cultural and socioeconomic characteristics.

o Adhering to safety training and protocols on a daily basis, and taking precautionary measure to ensure the safety and well-being of self, others.
o Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and protected health information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.

o Other duties as assigned by Program Director, Senior Director, or CEO.

Requirements
Minimum Qualifications:
· Education
o Minimum of Associate’s Degree in Nursing with relevant experience; Bachelor’s Degree in Nursing strongly preferred.
· Experience
o Minimum of 1 year of experience in nursing field; experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
· License and Credentials
o Current Compact or State of Texas RN license (in good standing)
o Current BLS for Healthcare Providers
o Must be up to date and willing to provide proof of vaccinations.
· Driving Requirements
o Must be at least 21 years old
o Reliable transportation
o Valid driver license
o Valid vehicle liability insurance
o Valid driver’s license
o Clean driving record
Minimum Knowledge and Skills:
o Minimum of 1 years of experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
o Experience with computer software to include Microsoft Office 365.
o A solid grasp of community and maternal-child health.
o Ability to work in a culturally sensitive manner with people of various backgrounds
o Ability to build strong relationships
o Ability to maintain appropriate ethical boundaries and professional relationships
o Must be flexible and adaptable, and be comfortable scheduling and rescheduling a day’s activities based on multiple factors.
o Must be detail oriented, organized, self-motivated, work well independently and on a team;
o Must have good written and verbal skills;
o Must have good critical thinking and proble-solving skills.
Working Conditions:
o This position may require occasional evening or weekend hours to accommodate the needs of families.
o Travel within the community and Bexar County to conduct home visits and attend meetings/trainings is required.
o Travel requirements for the position are approximately 98% local and 2% overnight per year. Overnight travel for trainings and conferences only.
o Occasionally, the role may entail additional responsibilities beyond the standard duties. This could involve tasks such as transporting items/supplies to and from office locations, department stores, client’s homes, or storage.
