Summary:
The case manager (CM) serves as a member of the multi-disciplinary mental health team responsible for providing recovery-oriented services to clients with severe and persistent mental illness, who may also present with co-occurring physical and/or developmental disabilities, and/or substance abuse issues. The CM will provide services related to improving Independent Living Skills and is responsible for providing support, guidance, and instruction to individuals to help them achieve and maintain independence in their daily lives.

Essential Responsibilities, include but are not limited to:
- Review/educate clients in areas identified in the service/treatment plan, such as medication management, healthy lifestyle, psychosocial needs, independent daily living skills, and self-management skills.
- Monitor and implement client needs, status, and preferences to ensure changes to the service/treatment plans are made, where necessary, to benefit the client.
- Collaborate with clients, the interdisciplinary team members, other providers, health plans, and other supports to ensure the service/treatment plan is being appropriately followed.
- Maintain efficient and consistent communication with Assistant Director of Mental Health Residential Program.
- Develop, utilize, and share clinical group curricula, keeping it up to date and adding new ideas or activities
- Provide clients with referral, linkage, and follow-up services to needed resources, such as PSH housing, entitlements, educational, vocational and guardianship services; Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
- Implement strategy to aid clients in improving or achieving independent living skills in a group or individual setting.
- Provide brief, supportive goal directed counseling to clients who may be experiencing difficulties with acquiring skills and/or based on assessed needs.
- Develop and facilitate therapeutic/recreational activities on site and in community.
- Maintain and update client’s pertinent information within Envision electronic care system as applicable.
- Attain/maintain assigned productivity targets, completing all documentation on a timely basis, including clinical documentation, service reporting, and billing Medicaid for services.
- Conduct assessments, including IM-CANs, to determine need/appropriateness for services, coordinating linkage to services identified. Assist with initial IM-CAN completion, as well as updates, as applicable.
- Assist Assistant Director with program development tasks such as designing workflows and procedures manuals.
- Other duties as assigned.
OVERALL JOB PERFORMANCE:
- At all times maintain confidentiality according to the Illinois Mental Health code and Confidentiality Act.
- Responsible for participating in required in-service training and community training seminars at the discretion of the Director. Pursue and keep abreast of current trends, methods and techniques in case management and counseling (minimum of 15 hours/year).
- Be prepared to present information from outside trainings to clinical staff.
- Develop, utilize, and share clinical group curricula, keeping it up to date and adding new ideas or activities.
- Attend/complete agency trainings as required.
- Participate in/attend committee meetings as applicable.
- Attend special events for clients and assist as needed.
- Inform supervisor of any condition that impacts the safety and wellbeing of clients served. Seek assistance as necessary to mitigate risk and seek out supervision as needed.
- Attend individual supervision with supervisor at scheduled time weekly to review and report on progress, productivity, and concerns.
- Demonstrate flexibility and adaptability with changes to program needs and schedule as the need arises.
SUPERVISORY RESPONSIBILITIES

QUALIFICATIONS
To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
EDUCATION, SKILLS, and/or EXPERIENCE
Bachelor’s degree in social work, counseling, psychology or related field, Master’s degree preferred. Must have minimum of one-year experience working within the social service arena, including internships. At least one-year case management experience, working with diverse populations with multi-faceted needs. Experience with mental health populations and knowledge of Medicaid rules preferred. Must have excellent written and verbal communication skills, decision making and problem-solving abilities, and must have exceptional computer and organizational skills.
Individuals must be able to work autonomously.
Have a valid driver's license and approval through agency's insurance carrier to drive agency's vehicles required.
Must pass Criminal Background Check and Nurse’s Aide Registry if applicable.
