Compensation

  • LSW/LPC: $55,000 – $68,000 annually

  • LISW/LPCC: $65,000 – $75,000 annually

  • Generous PTO available

  • Benefits begin the first of the month after hire

About the Role

Join Unison Health’s Assertive Community Treatment (ACT) Program and make a meaningful difference in the lives of individuals with severe and persistent mental illness (SPMI). This is a community-based clinical role that combines therapy, advocacy, and linkage services to help clients achieve stability and independence.

You’ll provide individual, group, and family therapy, as well as diagnostic assessments, under the supervision of an independently licensed clinician — all while working closely with a dedicated, multidisciplinary team.

Education & Experience

  • Bachelor’s degree in Social Work (Master’s preferred) OR Master’s degree in Counseling

  • Current Ohio licensure: LSW or LPC (LISW/LPCC preferred)

  • Minimum 1 year of experience working with SPMI clients

  • Valid driver’s license and ability to work flexible hours

  • Knowledge of:

    • Mental illness and treatment approaches

    • Individual and group counseling

    • Behavioral health assessment and crisis intervention

    • Chronic physical health issues affecting SPMI or SED populations

Primary Duties

  • Provide diagnostic services and therapeutic interventions to assigned clients

  • Deliver assertive community treatment services in line with state, agency, and program standards

  • Maintain timely, accurate documentation in the electronic health record

  • Ensure confidentiality and compliance with HIPAA

  • Participate in team meetings, training, and supervision to enhance service coordination and effectiveness
