Position Summary

The Medical Phone Triage Coordinator plays a key role in ensuring high-quality patient care through phone-based clinical support, education, care coordination, and follow-up. This position requires excellent clinical judgment, strong communication and problem-solving skills, and a compassionate, patient-centered approach.

Key Responsibilities

  • Assess patient phone calls and provide appropriate triage and intervention based on established guidelines, standards of care, and regulations.

  • Perform RN triage duties, including patient education, follow-up on abnormal lab results, transitional care management, and handling phone messages outside the CMA/MA scope.

  • Accurately document all patient interactions in the electronic medical record (EMR).

  • Manage tracking systems to ensure compliance with medical policies, guidelines, and health maintenance standards.

  • Participate in multidisciplinary teams to enhance patient outcomes and improve care processes.

  • Provide excellent customer service and professionally diffuse challenging situations.

  • Maintain confidentiality and comply with HIPAA standards in all interactions.

  • Manage, track, and complete emergency room (ER) follow-ups as needed.

  • Follow policies and procedures to resolve patient complaints and concerns.

Qualifications

  • Current State of Wisconsin Registered Nurse (RN) License or Nurse Compact License required.

  • 3–5 years of nursing experience preferred, ideally in an outpatient or family medicine setting.

  • Supervisory experience with an interdisciplinary team preferred.

  • Knowledge of quality improvement (QI) program implementation and maintenance preferred.

  • Proficiency with Microsoft Office and Epic EMR required.

  • Basic Life Support (BLS) Certification required.
