Description

Position Summary
The Day and CDS Manager must be able to connect with the community in a way that builds positive
relationships for those supported in the service. Utilize a systematic approach in maintaining the structure of
the day to day groups and skill building activities in the community. Day Habilitation, Community Development
Services and Introduction to Employment (Day Habilitation) help individuals learn new skills and keep the skills
they currently have related to their individual goals and outcomes. Services provides individuals with
opportunities to engage in community based activities that support building social connections (capital),
socialization, life-long learning, recreation, and personal development for the purpose of:

  1. Building and strengthening relationships with others in the local community who are not paid to be with the individual; and
  2. Learning, practicing, and applying skills that promote greater independence and inclusion in their community.

Duties and Responsibilities:

  • Manage direct reports who plan, prepare, and deliver community activities that facilitate active learning experiences both leisure and a focus on employment and job readiness
  • Assist Meaningful day instructors and specialists on how to provide systematic instruction and other teaching methods that help participants in small and/or large groups to learn skills to work in competitive integrated employment; such as, time management, strategies for completing tasks, interpersonal skills, effective communication, and how to problem-solve.
  • Utilize methods and ways to help participants have meaningful days in the community, training and support designed to maintain abilities and to prevent or slow loss of skills for individuals with declining conditions. Demonstrates commitment to employment as the first option for all people, regardless of ability.
  • Develop individualized schedules with each participant
  • Use and model person-centered practices, assisting people to make informed choices, develop strategies, take risks, follow through on responsibilities, and achieve their goals.
  • Assist in the development of the Person Centered Employment Plans to provide insight for future employment goals
  • Build trust and productive relationships with people supported, coworkers, and others through respectful, clear, concise and timely communication (verbally and in writing)
  • Facilitate teamwork, routine communication, and positive interactions among participants, within department, families and other departments to ensure that participant goals are met
  • Demonstrate sensitivity and objectivity while respecting the opinions, perspectives, customs, and individual differences of others
  • Follow ethical standards of practice (e.g., confidentiality, informed consent, etc), and recognize personal limitations
  • Hold monthly meetings with staff to gather important individualized information that pertains to the services provided.
  • Oversee the completion of Monthly calendars and distribute to teams of those served monthly
  • Act as a liaison between the day services and community to form and maintain meaningful community connections. Guide and support staff in developing partnerships and utilizing resources in the community that give participants the opportunity to develop and maintain skills related to community
  • membership, volunteer opportunities, retirement, and employment in integrated settings.
  • Attend meetings and webinars to stay informed of best practices and upcoming changes. Ensure center and programming continues to comply with the CMS Final Rule. Prepare, educate, and lead staff towards change to new DDA Meaningful Day Services and fee for service billing in LTSSMaryland.
  • Interview and assist with recruitment and hiring of new employees, provide on-the-job training, and terminations. Complete documentation as appropriate. Submit payroll biweekly. Monitor for staff compliance with training.
  • Provide teams with yearly progression of participants in services
  • Provide weekly reports of Groups and community activity
  • Perform Other Duties as assigned

 

Qualifications


  • At least 18 years old
  • GED or high school diploma
  • Supervision of direct staff experience
  • Technical knowledge to operate a computer and smartphone to perform essential job functions
  • Valid driver’s license with at least two years of driving experience and less than 3 points
  • Ability to effectively communicate both orally and in writing
  • Ability to lift 25-30lbs
