The Manager, Medical Case Management positions serve as part of the Ryan White Kentucky Care Coordinator Program (KCCP) Part B leadership team to help oversee daily efforts for nearly fifty Medical Case Managers (MCM) who support approximately 3,000 eligible active clients in the program. The Manager, Medical Case Management provides leadership in team workflow, caseload management, and data/reporting compliance to drive improved client outcomes within KCCP. Two positions available.
Essential Duties and Responsibilities
Oversee daily operations of approximately thirteen Medical Case Managers, ensuring delivery of comprehensive, client-centered services.
Manage client assignments and transfers to maintain 100% compliance with state-mandated caseload standards.
Develop and manage monthly rotation schedules to ensure consistent clinic coverage for new intakes at 550 Clinic.
Serve as lead resource for complex cases and crisis management; escalate critical issues as needed.
Submit monthly reports on team service delivery and care plan updates to program leadership.
Hold weekly meetings to support team performance and encourage professional growth.
Manage a caseload of 30+ clients, providing non-medical case management and adherence counseling.
Conduct acuity assessments and develop/update Individualized Care Plans (ICPs) every six months or as needed.
Complete biannual eligibility documentation, including HIV status, labs, residency, and income.
Document all services and client interactions in CareWare, including encounter details and frequency.
Coordinate care with healthcare providers, vendors, and community-based programs.
Recommend and support process improvements to enhance care delivery.
Review biweekly team activity reports to monitor productivity and data quality; provide feedback.
Lead CQI efforts and contribute to policy updates; participate in quarterly Ryan White Part B meetings.
Attend required training and staff meetings to support ongoing professional development.
Perform other duties as assigned.
Preferred Qualifications
Master’s degree in Social Work, Public Health, Public Administration, Epidemiology, Human Services, or a related health field, or equivalent experience.
Spanish language proficiency is a plus.
Minimum of 3 years of experience working with diverse populations and community stakeholders to address barriers to health equity.
Experience working specifically with individuals affected by HIV.
Familiarity with the Ryan White HIV/AIDS Program (RWHAP) is preferred.
Proficiency in Microsoft Office, particularly MS Word.
Compensation will be commensurate to candidate experience.
