Home Health Program Overview: 

RiverStone Health Home Health Services provides medical treatment for an illness or injury, to help patients recover, regain independence, and become as self-sufficient as possible. Home health care helps patients manage chronic conditions like heart disease, diabetes, pain management or chronic obstructive pulmonary disease (COPD) and provides personal care services.  Home Health Services may be requested by physicians, the patient or family member, or a hospital discharge planner.  Through consultation with the patient’s healthcare provider, Home Health Services develops a plan of care to meet the patient’s individual healthcare needs.  The team includes nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and certified home health aides.

Job Summary:

The RN Supervisor is responsible for the delivery of quality patient care through the nursing process of assessment, nursing diagnosing, planning, implementation, and evaluation. In tandem with the clinical manager and clinical nursing staff, the RN admissions nurse helps to oversee patient care, nursing case management, clinical documentation (OASIS), referral staff, and customer satisfaction with home health patient care services. The incumbent brings content expertise and knowledge surrounding the program elements, deliverables, and metrics. The work environment is typically fast paced.  Work is performed to ensure efficiency of program delivery, incorporating integrity and innovation to ensure a high level of customer service.

Essential Functions/Major Duties and Responsibilities: 

Lead RN/QAPI                                                                                                                                                                                                                               50%

  • Coordinates the nursing team, including scheduling, workload management, and coverage to ensure efficient service delivery
  • Provides clinical input for referrals, admissions, and intake processes, ensuring timely initiation of care.
  • Participates in regular interdisciplinary team conferences to promote collaboration, information exchange, and integration of care plans.
  • Oversees accurate and timely documentation within the electronic medical record, ensuring compliance with regulatory and agency standards.
  • Provides clinical guidance and support for nursing staff in complex cases and decision-making.
  • Directs the work of nursing staff, establishes schedules, reviews timecards, and ensures adherence to policies and procedures.
  • Conducts performance evaluations, provides coaching and corrective action as needed, and collaborates with Human Resources on nurse recruitment, onboarding, and professional development.
  • Promotes teamwork, accountability, and professional growth among nursing staff.
  • Provides leadership in developing, implementing, and evaluating home health quality improvement and compliance initiatives, including QAPI activities.
  • Identifies areas for home health improvement, gathers and analyzes data, and reports findings to Program Directors and staff.
  • Leads QAPI projects, establishing measurable goals and performance indicators using the Plan-Do-Study-Act (PDSA) Model for Improvement.
  • Educates staff on quality improvement principles and promotes adherence to evidence-based best practices.
  • Monitors compliance with Medicare Conditions of Participation, accreditation standards, and internal policies, identifying risks and assisting with corrective action planning.
  • Reviews and investigates incident reports, ensures appropriate follow-up, and implements preventive and corrective strategies.
  • Provides clinical oversight for utilization review and care management activities with payers and third-party entities.
  • Supports ongoing accreditation and regulatory readiness through audits, compliance reviews, and survey preparation
  • Participates in leadership meetings, providing quality, compliance, and clinical updates.
  • Serves in the leadership on-call rotation as assigned.

Admissions/Direct Skilled Care                                                         45%                         

  • Provides direct skilled nursing care to support case managers in the Home Health and Hospice programs.
  • Completes patient admissions and initiates individualized plans of care in collaboration with physicians and the interdisciplinary team.
  • Reassesses patient needs and revises the plan of care to ensure effective, patient-centered outcomes.
  • Follows a plan of care that identifies, prioritizes, and meets the physical, emotional, and educational needs of patients and families.
  • Administers medications, treatments, and interventions in accordance with physician orders and nursing best practices.
  • Monitors, interprets, and evaluates patient symptoms, progress, and treatment responses; implements appropriate clinical actions and updates care plans accordingly.
  • Provides education to patients and families regarding disease management, medication safety, preventative care, and discharge planning.
  • Utilizes knowledge of age-specific and cultural considerations to tailor care and education to individual needs.
  • Supervises and evaluates the work of Licensed Practical Nurses (LPNs) and Certified Home Health Aides (CHHAs), ensuring high standards of care.
  • Communicates effectively with physicians, families, and interdisciplinary team members to ensure seamless coordination of care.
  • Evaluates patient outcomes and identifies areas for improvement or intervention as needed.
  • Maintains timely and accurate documentation in the electronic medical record per agency standards.
  • Demonstrates working knowledge of OASIS-E data collection, quality measures, and reimbursement requirements.
  • Participates in on-call rotation to ensure 24-hour nursing coverage for patients, carrying designated communication devices as required.
  • Ensures compliance with infection control, safety, HIPAA, and agency policies.
  • Participates in performance improvement activities and contributes to quality initiatives that enhance patient outcomes and program performance.
  • Develops and maintains effective working relationships with patients, families, staff, and community partners.
  • Responds promptly and courteously to inquiries, feedback, and concerns from patients, families, and team members.
  • Attends continuing education and training sessions to remain current on nursing practice standards, quality improvement methodologies, and regulatory changes.

Education and Experience:

Minimum Qualifications

  • Associate degree in Nursing.
  • 3 years nursing experience
  • Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work.

Preferred Qualifications:

  • Bachelor of Science in Nursing
  • Two years of case management
  • Knowledge of nursing process and home care regulations preferred.

Certificates, Licenses, Registrations:

  • Current license as a Registered Nurse in Montana.
  • Valid State of Montana Driver’s License and vehicle insurance (if Hospice, Home Care, and Public Health)
  • BLS and CPR certification required

Physical Demands and Working Conditions: 

  • Required to stand, walk and sit; talk or hear, both in person and by telephone; use hands to finger, handle or feel objects or controls; reach with hands and arms.
  • Regularly required to stoop, kneel, bend, crouch and lift up to 25 pounds frequently and 50 pounds occasionally; expected to ask for help from a colleague or utilize a lift when assistance is needed.
  • Specific vision abilities required by this job include close vision, distance vision, depth perception, color vision and the ability to adjust focus.
  • Ability to travel and use own personal vehicle. Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, other vehicles, etc. Travel occurs in all weather conditions, including extreme heat and cold.
  • Risk of needlestick injury is possible. 
  • Position is regularly exposed to biological pathogens and contagious diseases where Personal Protective Equipment (PPE) and special precautions must be taken due to reduce exposure and mitigate risk.
  • Will be exposed to home environments that may include allergens such as smoke, pets, etc.
  • Create and maintain a safe/secure working environment by adhering to safety, security, and health requirements. Integrate injury, illness, and loss prevention into job activities by attending any necessary training and implementing best practices.

  • Work is structured by regulations and established nursing practices. The position makes independent professional judgements regarding patient health.  Decisions directly affect the quality of services provided to the public.

  • Contacts occur both inside and outside RiverStone Health and typically are for the purposes of rendering services to patients to gather information and provide education and instruction.
  • Communication with others is a critical element of the position because the purpose of the contract may be to negotiate matters involving significant issues of importance.
