KEY RESPONSIBILITIES
Hospital Liaison & Critical Care Transitions (~50%)
- Serve as the primary clinical contact for NICU/PICU teams, case managers, and discharge planners.
- Guide referral-to-admission processes, ensuring patient readiness and compliance with Family First standards.
- Build and maintain strong hospital relationships through expertise, reliability, and professionalism.
- Act as the escalation point for transitions at risk, resolving issues and preventing breakdowns.
- Represent Family First at hospital meetings, caregiver presentations, and community events.
Clinical Education, Training & Quality (~50%)
- Partner with VPCs to design, deliver, and oversee all CCT education, including equipment, devices, and competency assessments.
- Coordinate CCT admissions from referral through start of care, ensuring alignment with clinical leadership and offices.
- Partner with VPCs to ensure timely follow-up and reporting of changes in client condition.
- Track and analyze all CCT admissions to monitor performance and identify opportunities for improvement.
- Mentor Directors of Nursing (DONs), Clinical Managers (CMs), and field staff to build confidence and skill in critical care.
- Support broader education initiatives such as preceptor programs, accreditation preparation, and new market readiness.
- Evaluate training effectiveness through measurable outcomes in quality, safety, and patient satisfaction.
- Lead or support onboarding and continuing education for new offices, acquisitions, and growth initiatives.
Leadership & Collaboration
- Bridge clinical, business development, and operational teams to ensure aligned execution.
- Provide insights to the CNO and CDO from hospital relationships, referral patterns, and field education.
- Champion continuous improvement in referral-to-admission processes to enhance safety and efficiency.

ROLE QUALIFICATIONS
EDUCATION & EXPERIENCE
- Active FL RN license (multi-state compact or eligibility)
- Significant pediatric critical care experience (NICU, PICU, or complex pediatric conditions)
- Proven leadership and clinical education experience
- Strong relationship-building and communication skills with hospital and healthcare stakeholders
- Excellent organizational and presentation skills
- Willingness to travel frequently across Florida and Texas
PREFERRED
- Pediatric homecare or home health experience
- Experience in quality improvement, accreditation, or new market launches
- BSN or higher; advanced certifications in pediatrics, education, or critical care
WORKING CONDITIONS/EQUIPMENT USE
Travel
Approximately 75% of travel is required. Typically, WFH on Mondays; travel Tuesday–Friday for field, hospital, or training activities.
