Position Summary
We are seeking a motivated and detail-oriented LPN Case Manager to join our remote team. In this role, you’ll coordinate medical care for injured or ill individuals, working closely with healthcare providers, insurance carriers, employers, and employees. Your clinical expertise and case management skills will help individuals return to optimal work performance.

Key Responsibilities

• Facilitate medical treatment for work and non-work-related injuries and illnesses.
  
• Act as a liaison among care providers, insurers, employers, and employees.
  
• Monitor case progress and report updates to employers and carriers.
  
• Support cost-effective care strategies while maintaining quality outcomes.
  
• Complete initial follow-up assessments within 24 hours.
  
• Maintain accurate records and meet performance benchmarks.
  
• Stay updated on clinical best practices and participate in continuing education.
  

Qualifications

• Current, valid LPN licensure (multi-state licensure preferred).
  
• Minimum 2 years clinical nursing experience required.
  
• ER or ICU experience preferred.
  
• Previous case management and workers’ compensation experience is a plus.
  
• Strong organizational, communication, and interpersonal skills.
  
• Proficient in Microsoft Office and typing (minimum 50 wpm).
  
• Ability to work independently and manage priorities effectively.
  
• Must pass background check and drug screening as applicable.
  

Work Environment & Schedule

• Work-from-home position with a requirement to maintain a professional environment.
  
• Typical shifts range from 8–12 hours; flexibility for occasional evening, night, weekend, or holiday hours may be needed.
  
• Participation in monthly staff meetings and training required.
