Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies.
We are seeking a full-time, 40-hour PTA to join the “Paul Revere” team. While most of your time will be spent working in Roxbury, Dorchester and Jamaicia Plain, the entire team also supports Mission Hill, MA.
While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible!
The desired schedule is Monday through Friday. Anticipated daily volume is 6-7 visits per day plus chart review, visit preparation, documentation, and care coordination.

Job Summary

Qualifications
Provide physical therapy services, under the direction of a registered physical therapist to patients in their place of residence , who have physical impairments, resulting from injury, disease, disability or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.

Qualifications and Experience
• Two years experience in acute care, rehab or homecare as a Physical Therapy Assistant
• Licensed as a Physical Therapist Assistant in Massachusetts
• Proficient computer skills and ability to learn other software applications
• Strong verbal, written and electronic communications skills
• Travel required within PHH’s geographic area
• Ability to work with various computer software required and knowledge of hand-held patient care computer devices
