Wentworth Health Partners Seacoast Pulmonary Medicine offer a broad spectrum of specialized services for patients with all types of pulmonary and respiratory diseases, including obstructive and restrictive lung diseases, respiratory failure, lung cancer, occupational lung disease and more.


 

Job Summary

We are seeking a full-time Nurse Navigator (40 hours per week), Monday through Friday, between 8:00am. and 5:00pm. The primary focus of this role will be providing direct support to COPD patients through both in-person visits and phone triage. The Nurse Navigator will also assist with cross coverage for pulmonary patients as needed.

This position may offer the opportunity for partial remote work in the future.

Responsible for coordinating the complex care needs of patients and is the primary patient contact for service access, diagnostics, treatment activities, schedules, patient and family education, and multidisciplinary collaboration.

Essential Functions
-In collaboration with the interdisciplinary team, help create a tailored health care plan for assigned patients.
-Develop an individualized plan with the MD for appropriate services for the patient.
-Reports pertinent observations and reactions regarding clients to the appropriate person (i.e., primary care provider, social worker, or Director).
-Assesses and coordinates patients' discharge planning needs with members of the healthcare team.
-Resolves patient problems and needs by utilizing multidisciplinary team strategies.
-Educates patients regarding how to navigate the health system.


 

Qualifications

Education
Bachelor's Degree Nursing required

Can this role accept experience in lieu of a degree?

Licenses and Credentials
Registered Nurse [RN - State License] - Generic - HR Only preferred

Basic Life Support [BLS Certification] - Data Conversion - Various Issuers preferred

Experience:
Experience in an acute care setting and/or provider practice and/or experience in a community-based role or with discharge/transitional planning background 2-3 years required

Knowledge, Skills and Abilities
- Strong attention to detail and organization.
- Strong cultural awareness competencies.
- Proficiency in Microsoft Office.
- Maintains good communication skills.
- Readily adapts to change and welcomes new approaches when circumstances demand it.
