Full-Time, Non-Union, Exempt Position
$23.92 per hour, 40 hours per week
Monday-Friday, 8:00am-5:00pm
BASIC FUNCTION OF OUTREACH AND INTERVENTION MANAGER:
Supervise Social Services Coordinator collaboration with the participants in developing an integrated service plan that includes the participant’s short- and long-term goals and agency interventions.
RESPONSIBILITIES OF OUTREACH AND INTERVENTION MANAGER:
- Supervise CT Project Longevity Street Outreach and Intervention Program will employ interconnected strategies to ensure programmatic effectiveness.
- Promote and demonstrate NOI’s mission, vision and values through both behavior and job performance on a day to day basis
- Enter and analyze data for reports that are filed with funding sources and other agencies as required on a monthly and quarterly basis
- Works with leadership and staff to create trainings, resources, policies and procedures that ensure good quality care
- Develop prevention and intervention outreach strategies with outreach partners
- Administrative supervision of client files, workflow, and day to day case conferences
- Build community partnerships by networking and partnering with local police, including intel meetings, and shooting reviews
- 24/7 on-call operative support by responding to significant incidents by being present in the neighborhoods, at the hospitals, and connecting with the associates of shooters and victims as soon after the event as possible
- Collaborating with other Project Longevity staff in reviewing referrals, including the Level of Service Inventory (LSI) and Veoci data collection
- Collaborating with other Project Longevity staff in conducting intake, screening, and assessments;
- Building positive relationships with Project Longevity clients and other community members and encouraging positive decision making; e. Facilitating community involvement and community-based programming and services
- Implement file recordkeeping system and client confidentiality procedures in accordance with Program funding source and CMS
- Coordinate the enrollment of families into social service programming in support of client goals. This may include application assistance/review and/or authorization of direct benefit payments.
- Encourage clients to develop their own strengths including identification and practicing of needed skills
- Other related duties as assigned
Requirements
REQUIREMENTS OF OUTREACH AND INTERVENTION MANAGER:
Education:
Bachelor’s Degree Human Services field requirement or similar combination of education and/or experience.
Experience:
- Excellent written and verbal communication skills and knowledge of communication principles and practices.
- Knowledge of social media tools: Facebook, Twitter, Knowledge of website management.
- Ability to prioritize, multi-task and follow through with minimal direction.
- Flexibility in varied job assignments.
Job Skills:
- Knowledge of relevant State and Federal agency programs, including eligibility and reporting requirements
- Knowledge of community needs and resources; ability to maintain productive community relationships
- Oral and written communication skills; interpersonal skills
- Customer service, organizational and problem-solving skills
- Ability to counsel clients with respect and dignity; tenacity and commitment to program goals
- Recordkeeping and computer skills
- Bi-lingual a plus; ability to work with diverse populations
Licenses /Certifications:
Current CT Driver’s license / current CT required insurance/ own transportation.
