Family Service Coordinator (FSC) - Hopes Portage Shelter Specialist
Monday – Friday 7-3:30 and 2-10:30
1 FTE (40 hours/week)
SUMMARY
The FSC - Hopes Portage Shelter Specialist is responsible for coordinating and providing support services to families who are actively housed in Hopes Portage family shelter. They must be able to work effectively with a diverse group of clients and be able to advocate for their needs. They must also have a thorough understanding of community resources and be able to connect clients with appropriate services. Family Service Coordinators may also be responsible for developing and implementing programs and services, as well as monitoring and evaluating their effectiveness. The FSC will be located at the Hopes Portage Shelter in Prior Lake MN. The intensity of service delivery will vary according to individual needs and progress toward achieving self-defined goals as set forth by client-centered practices.

- Maintain Client Confidentiality
- The Specialist challenges, assists, councils, advocates for, and empowers clients to become self-supporting
- Maintain all records according to agency/program guidelines
- Utilize HMIS and Client Track.
- Complete assessments with families to help them in their journey
- Be knowledgeable of all CAP Agency programming and community resources; collaborate with staff and external sources for the coordination of services.
- Participate on a CAP Agency housing team to implement outreach, recruitment and development of myriads of community partnerships for all housing programs to bolster the services and support for the clients we serve
- Attend and participate in meetings, workshops, conferences and community events as requested, and regularly attend Heading Home Scott-Carver Shelter planning meetings to assist in the creation of an effective, homeless response system.
- Participate in a CAP Agency Community Team to implement outreach, recruitment, and development of community partnerships for agency programs
- Attend relevant community meetings

MINIMUM QUALIFICATIONS
Education, Training and Experience Guidelines
2-year degree in social work or related field plus one year of associated experience or equivalent combination of education and experience. Experience working with households experiencing homelessness is a must. Experience working with housing programs preferred. Have an awareness of community programs and resources. Must have the ability to implement a strength-based and empowering approach to service delivery.
Knowledge of:
- Must be able to translate and effectively deliver grant funded program information to program applicants, CAP staff, County staff and other agency partners.
- Must possess knowledge to understand the challenges of clients with mental health, chemical dependency, and behavioral issues.
- Applicable policies, procedures and regulations covering specific areas of assignment.
- Principles and practices of conflict resolution, customer service and outreach.
- Customized CAP and state agency software applications and procedures.
- Local community resources and regional community services programs.
- Principles of record keeping and record management.
- Personal computers utilize standard and specialized software.

LICENSE AND CERTIFICATION REQUIREMENTS
A valid Minnesota State Driver’s License, proof of insurance and review of Motor Vehicle Record (MVR) report is required.
PHYSICAL DEMANDS AND WORKING ENVIRONMENT
Work is performed in a standard office environment which requires occasionally lifting such articles as file boxes or heavier materials with help from others and/or lifting and carrying light objects frequently. Mobility to drive a motor vehicle to attend meetings. A job in this category may require walking or standing to a significant degree or may involve sitting most of the time with long periods of computer work and heavy phone usage.
