Summary
The Hospital Team/Referral Center Team Manager (RCTM) is responsible for the oversight and management of a team of hospital liaisons, whose primary responsibilities include ensuring professional management, in accordance with their hospice Plan of Care, is maintained for existing VIA patients while in the inpatient setting; assessing eligibility, presenting services, coordinating an appropriate disposition plan in collaborative effort with hospital discharge planners, and providing ongoing education and consultation for healthcare professionals and potential consults within the hospital systems. Generally, the RCTM is responsible for managing the day-by-day activities of an interdisciplinary team including supervision, resource management, scheduling, and assignments. The RCTM ensures that all patient care is provided in accordance with established organizational procedures and regulatory requirements. The RCTM aids in recruitment process, develops, supervises, and trains clinical and office staff. The RCTM ensures exceptional care through the professional development and coaching of staff as well as ensures adherence to clinical documentation expectations. The RCTM also actively participates in ongoing process improvement.
Hospital Liaison (HL) - Responsible for ensuring professional management, according to the hospice plan of care, of new referrals and existing HPCCR patients while in the inpatient setting. Responsible for maintaining and developing relationships with hospital-based referral sources in collaboration with the Marketing and Community Awareness teams. The HL team manager also maintains responsibility for the new and ongoing hospital credentialing requirements of the clinical team.
Essential Functions
- Responsible for oversight and direction for an interdisciplinary hospice team.
- Ensures that clinical care is effective and meets the needs of patients and families.
- Provides supervision and evaluation including timely performance appraisals of team members.
- Formulates and implements corrective actions because of evaluations or when performance issues are observed. Assists Sr. Director and Human Resources with the termination of employees in accordance with performance standards, staff policies, and job descriptions.
- Assists with hiring, orientation and mentoring of new employees.
- Ensures development and revisions of the plan of care through IDT meetings and other formal and informal planning activities.
- Ensures that the service is provided within the structure, policy and regulatory environment required by VJA.
- Ensures that discharge decisions are based on meaningful assessment information and follow appropriate procedure.
- Ensures that documentation meets all standards as designated by VIA.
- Provides problem solving and palliative care and symptom management consultation to team members and peers.
- Complies with all expectations of quality improvement activities to continuously monitor, evaluate, and improve team performance.
- Ensures that all clinical staff function in accordance with established procedures and legal requirements.
- Collaboratively works with team within EMR to ensure certification/recertification process is completed per organizational polices and regulatory requirements.
- Provides back-office support within EMR per organizational expectations.
- Assumes responsibility for the effective administration of team functions.
- Actively reviews available reports and statistical data to manage the team effectively and efficiently.
- Ensures that all patient care meets professional, organizational, regulatory, licensure, and accreditation standards through on-going monitoring and improvement activities.
- Assumes responsibility for professional development and staying abreast of current trends in the healthcare field.
- Ensures the timely incorporation of new information, policies and/or procedures into practice.
- Ensures accuracy of and approves time sheets/payroll summary forms and paid time off requests for direct reports.
- Provides ongoing monitoring for compliance with internal initiatives.
- Recommends modifications of existing or development of new policies and procedures based on practice patterns and needs.
- Ensures continuity of patient care and Referral Center operations during times of regularly assigned staff absences.
- Maintains professional relationships with other providers, patients and families, and vendors.
- Maintains existing partnerships between hospice and community providers, referral sources, and peer hospice and palliative care providers.
- Responds to customer concerns and facilitates prompt resolution of issues. Make appropriate changes in practice based on concerns.
- Utilizes feedback from long term care and physician practices to provide exceptional customer service.
- Ensures that problems and questions are effectively addressed, researched, and resolved.
- Receives and responds to text messages/e-mails/voice mails as necessary.
- Presents and maintains a highly professional image.
- Act as community resource and expert on adult and/or pediatric end of life care.
- Assumes responsibility for establishing and maintaining effective working relationships with leadership and staff in other departments.
- Attends meetings and participates actively in internal organizational activities. Reports and acts on potential issues for improvement, participates in work groups, and participates in quality assurance audits as needed.
- Keeps other leadership and supervisors well informed of area activities and of any significant problems.
- Cross trains with other Team Managers to ensure continuity of leadership as needed.
May perform other duties as required.
Qualifications
Minimum Qualifications
- Bachelor’s Degree in related field required; master’s degree preferred. Or the equivalent combination of education, technical certifications, training, and work experience.
- Minimum 2 years’ related experience required with increasing management responsibilities. Related experience includes home health/hospice, oncology, and medical surgical.
- Proficiency in Microsoft Office is required.
- Active Registered Nurse license in NC/SC required.
