About the Job:
PURPOSE STATEMENT:The Case Manager supports the department by providing case management to the different units within the facility. Manages the patient’s cases with a solid background of clinical knowledge and expertise and may provide some therapeutic services to patients. Responsible for the overall discharge and aftercare planning needs and placement of the patient. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.

As a Case Manager, you’ll play a key role in guiding individuals through every phase of their recovery journey — connecting them with resources, advocating for their needs, and helping them transition toward independence and stability.
What You’ll Do
Every day, you’ll make a real difference by:
Developing and implementing service plans that are personalized, goal-driven, and rooted in recovery.
Collaborating with patients, families, and treatment teams to create meaningful, effective care plans that support long-term wellness.
Maintaining thorough and accurate clinical records, documenting assessments, interventions, progress, and discharge plans.
Coordinating care with medical providers, therapists, and community partners to ensure continuity of treatment.
Providing education and guidance to patients and families on available clinical options, community resources, and aftercare services.
Supporting discharge and aftercare planning—from arranging follow-up appointments to coordinating transportation and community placements.
Responding to crises using trauma-informed intervention techniques to de-escalate and stabilize situations.
Facilitating small group sessions on topics such as coping skills, anger management, social skills, and independent living.
What You Bring
Bachelor’s Degree in Behavioral Health, Psychology, Social Work, Business Administration, or related field (preferred)
1+ year of experience in behavioral health, case management, or utilization review (preferred)
Strong organizational and communication skills, with the ability to engage compassionately with patients and families
Passion for helping others succeed in recovery and reintegrate into the community
