The hospice navigator will work with hospital staff and the General Inpatient (GIP) team to quickly identify potential hospice patients and work to get those patients prepared for a successful transition to home-based hospice care. Bachelor’s degree in nursing, Master’s Degree Preferred

Home-based hospice care experience required

Five+ years nursing experience within a hospice agency or related community health program which included care of the sick at home.

Licensure, Certification, Registration

Current discipline licensure to practice in the State of Connecticut.
